射干麻黄汤治疗支气管哮喘急性发作期的治疗进展浅析

摘　　要：支气管哮喘是肺部常见的缠绵难愈的疾病之一,其病位主在肺,与中焦的脾胃和肝及下焦的肾、大肠密切相关。其近些年的发病率和致死率都在持续上升,西医认为支气管哮喘的产生是多种细胞(嗜酸性粒细胞、中性粒细胞、气道上皮细胞等)及细胞因子共同参与而导致的慢性气道炎症异质性疾病,西医在治疗方面多用糖皮质激素和β受体激动剂,虽有一定疗效,但在完全康复及预后方面还有待改善,并且长期反复发作,严重者会继发慢性阻塞性肺疾病、肺气肿等疾病。故备受广大人士关注,且临床上急性发作期的频次也在增加,时时刻刻在威胁着人类的生活质量,损害着人们的经济水平,使患者承担大量的医疗费用。随着中医的不断深入研究发现,因本病多在清晨或者夜间发病,好发季节是初春和秋冬,故以寒哮证多见,症见喉中哮鸣有声,呼吸困难,甚至张口抬肩,不能平卧,且会伴有咳嗽咳痰。射干麻黄汤是治疗寒哮证的经典代表方,通过各中药之间的配伍,升降同施,温润并用,宣肺散寒,化饮止咳,对支气管哮喘发挥着温肺散寒、化痰理气之功效,临床治疗效果较好,故笔者从本病的中医病因病机及射干麻黄汤的药物药理作用、作用机制等方面进行浅析。Bronchial asthma is one of the common lingering and difficult diseases of the lung,its main location is in the lung,and is closely related to the spleen,stomach and liver of the middle-jiao(中焦)and the kidney and large intestine of the lower-jiao(下焦).In recent years,its morbidity and mortality have been on the rise.Western medicine believes that bronchial asthma is a heterogeneous disease of chronic airway inflammation caused by the co-participation of a variety of cells(eosinophils,neutrophils,airway epithelial cells,etc.)and cytokines.Western medicine mainly uses glucocorticoids andβ-receptor agonists in treatment,which have certain efficacy.However,it still needs to be improved in terms of complete recovery and prognosis,and long-term recurrent attacks,and severe patients will have secondary diseases such as chronic obstructive pulmonary disease and emphysema.Therefore,it is concerned by the majority of people,and the frequency of clinical acute attacks is also increasing,which is threatening the quality of human life,damaging people’s economic level,and bringing a large number of medical costs.With the continuous in-depth study of traditional Chinese medicine,it is found that the disease mostly onsets in the morning or at night,the season is in early spring and autumn and winter,so it is mainly common with cold wheezing syndrome,the symptoms include wheezing in the throat,breathing difficulties,and even difficulties in opening mouth and lifting shoulder,being unable to lie flat,and it is accompanied by cough and sputum.Shegan Mahuang decoction(射干麻黄汤)is a classical representative prescription for the treatment of cold wheezing syndrome.Through the compatibility of various TCM medicines,the application of regulaing the ascending and descending,warming and moistening,promoting the lungs and dissipating cold,and eliminating phlegm and relieving cough,it plays a role in achieving the effect of warming the lungs and dispelling cold,eliminating phlegm and regulating Qi(气)for bronchial asthma,an
