引阳入阴穴位推拿联合中药沐足对脑梗死后失眠患者的疗效观察

摘　　要：目的:评价引阳入阴穴位推拿联合神宁方中药沐足治疗脑梗死后失眠患者的临床疗效,为今后临床治疗脑梗死后失眠提供更好的治疗方案。方法:选取2019年10月―2022年1月上海市嘉定区中医医院内科收治的80例脑梗死后失眠患者为研究对象,将符合诊断标准及纳入标准的脑梗死后失眠患者按随机数字表法分为两组,分别为治疗组和对照组,每组40例。对照组给予艾司唑仑片口服治疗,治疗组在对照组基础上给予引阳入阴穴位推拿联合神宁方中药沐足治疗,所有患者均治疗14 d。对比两组患者治疗前后临床疗效及匹兹堡睡眠质量指数(Pittsburgh Sleep Quality Index,PSQI)、焦虑自评量表(Self-rating Anxiety Scale,SAS)、抑郁自评量表(Self-rating Depression Scale,SDS)和美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分变化情况并进行统计分析,比较两组安全性。结果:治疗后,治疗组和对照组总有效率分别为89.5%(34/38)和69.4%(25/36),治疗组的疗效优于对照组(P <0.05)。与本组治疗前比较,治疗组和对照组患者的PSQI、SAS、SDS和NIHSS评分治疗后均显著降低(P <0.01)。与对照组同期比较,治疗组治疗后PSQI、SAS、SDS和NIHSS评分均明显降低(P <0.01)。两组患者均无明显不良反应发生。结论:引阳入阴穴位推拿联合神宁方中药沐足疗效确切,能有效改善脑梗死后失眠患者睡眠质量及伴随的焦虑、抑郁情绪,促进神经功能恢复,且无明显不良反应,对提高临床疗效有积极正向作用。Objective:To evaluate the clinical efficacy of acupoint massage based on Yin-yang(阴阳)regulation combined with TCM medicine foot massage for post-stroke insomnia,so as to provide a better treatment plan for the clinical treatment of post-stroke insomnia.Methods:A total of 80 patients with insomnia after cerebral infarction admitted to Department of Internal Medicine,Jiading District Hospital of Traditional Chinese Medicine from October 2019 to January 2022 were selected as the study subjects.Patients with insomnia after cerebral infarction who met the diagnostic criteria and inclusion criteria were divided into two groups according to random number table method,namely,with 40 cases in the treatment group and 40 cases in the control group.The control group was given oral Esazolam tablets,and the treatment group was given acupoint massage based on Yin-yang regulation combined with Shenning prescription(神宁方)foot massage on the basis of the treatment in the control group.All patients were treated for 14 days.The clinical efficacy and the score changes of Pittsburgh Sleep Quality Index(PSQI),Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS)and National Institutes of Health Stroke Scale(NIHSS)before and after treatment were compared between the two groups,and statistical analysis was carried out to compare the safety in the two groups.Results:After treatment,the total effective rate in the treatment group and the control group was 89.5%(34/38)and 69.4%(25/36),respectively,and the curative effect in the treatment group was better than that in the control group(P<0.05).Compared with those before treatment,PSQI,SAS,SDS and NIHSS scores in the treatment group and the control group were significantly decreased after treatment(P<0.01).Compared with those in the control group,PSQI,SAS,SDS and NIHSS scores in the treatment group were significantly decreased after treatment(P<0.01).No obvious adverse reactions occurred in both groups.Conclusion:Acupoint massage based on Yin-yang regulation combined with
