基于“气血理论”探讨补阳还五汤在治疗糖尿病周围神经病变中的作用机制

摘　　要：糖尿病周围神经病变(Diabetic Peripheral Neuropathy,DPN)是糖尿病的主要并发症之一,它是一种多神经病变,以远端、对称性的肢体感觉异常为主,通常以肢体的麻木、刺痛、烧灼感及感觉减退甚至缺失等为表现,其发病率呈现逐年递增的趋势,严重影响患者的生活质量。气血理论乃中医理论的重要组成之一,气与血是构成人体的两大基本物质,维持人体的生命活动。气血乃人体之精所化,气属阳,血属阴,二者互根互用。文章从气血理论出发,探讨补阳还五汤在治疗DPN中的作用机制。补阳还五汤乃治疗气虚血瘀中风之名方,具有“补气、活血、通络”之功效。中医学认为DPN属于“痹病”“痿病”等的范畴。消渴日久,耗伤气血、损伤阴阳,最终导致气虚而血行不畅,瘀阻而脉络不通,故气虚血瘀乃本病的主要病机。气虚则血行不畅,瘀阻则脉络不通,治疗上以补气为本,活血化瘀为标,标本兼施,使气旺、瘀消、络通,诸症向愈。目前大量的基础实验和临床研究从分子、细胞、器官以及免疫学等多个层面对补阳还五汤加以研究,并取得了一定的成绩,充分肯定了补阳还五汤在临床各科疾病中的治疗作用。课题组在西医治疗的基础上联合补阳还五汤,观察其临床疗效显著,且安全可靠,值得推广应用。Diabetic peripheral neuropathy(DPN)is one of the main complications of diabetes.It is a multi-neuropathy,which is mainly characterized by distal and symmetrical limb paresthesia,usually manifested by numbness,tingling,burning sensation,loss or even loss of sensation,etc.,and its incidence is increasing year by year,which seriously affects the quality of life of patients.The theory of Qi(气)and blood is one of the important components of the theory of traditional Chinese medicine.Qi and blood are the two basic substances that constitute the human body and maintain the life activities of the human body.Qi and blood are the essence of the human body,Qi belongs to Yang(阳),blood belongs to Yin(阴),the two are mutual dependent and interacting.Based on the theory of Qi and blood,this paper discusses the mechanism of Buyang Huanwu decoction(补阳还五汤)in the treatment of diabetic peripheral neuropathy.Buyang Huanwu decoction is a famous prescription for treating Qi deficiency and blood stasis and stroke,and it has the effect of“replenishing Qi,promoting blood circulation and clearing collaterals”.In traditional Chinese medicine,diabetic peripheral neuropathy belongs to the categories of“arthralgia syndrome”and“impotent syndrome”.Diabetics that lasts a long time will injure Qi and blood,damage Yin and Yang,which eventually leads to Qi deficiency and stagnant blood flow,blood stasis or even collaterals obstruction,so Qi deficiency and blood stasis is the main pathogenesis of the disease.Qi deficiency means poor blood circulation,and blood stasis means blocked collaterals,the treatment is based on replenishing Qi,promoting blood circulation and removing blood stasis,and targetting both the root causes and symptoms to dissipate blood stasis and unblock collaterals,and all symptoms will disappear.At present,a large number of basic experiments and clinical studies have studied Buyang Huanwu decoction from the aspects of molecular,cellular,organ and immunology,and have made certain achievements,which fully
