摘 要:目的探讨碎裂心电图QRS波联合病理性Q波诊断急性心肌梗死(AMI)的价值。方法选取我院2020年1月至2022年12月收治的158例冠心病患者予以心电图检查。以病理诊断作为金标准,计算碎裂QRS波、病理性Q波及碎裂QRS波联合病理性Q波诊断AMI的灵敏度和特异性。结果碎裂QRS波诊断AMI的特异性、灵敏度、准确度、AUC分别为57.97%、50.56%、53.80%、0.540(0.461~0.619),病理性Q波诊断AMI的特异性、灵敏度、准确度、AUC分别为63.77%、58.43%、60.76%、0.611(0.534~0.688),碎裂QRS波联合病理性Q波诊断AMI的特异性、灵敏度、准确度、AUC分别为66.67%、74.16%、70.87%、0.704(0.632~0.776)。碎裂QRS波联合病理性Q波诊断AMI的AUC高于碎裂QRS波和病理性Q波(Z=5.706,P<0.05;Z=4.251,P<0.05)。结论碎裂QRS波联合病理性Q波诊断AMI具有较高的特异性、灵敏度和AUC,诊断效果较好。Objective To analyze the value of fragmented ECG QRS wave combined with pathological Q wave in the diagnosis of acute myocardial infarction(AMI).Methods 158 patients with coronary heart disease admitted to our hospital from January 2020 to December 2022 were selected and given ECG.Taking pathological diagnosis as the gold standard,the sensitivity and specificity of fragmented QRS wave,pathological Q wave,and fragmented QRS wave combined with pathological Q wave in the diagnosis of AMI were calculated.Results The specificity,sensitivity,accuracy and AUC of fragmented QRS wave in the diagnosis of AMI were 57.97%,50.56%,53.80%,and 0.540(0.461-0.619),respectively.The specificity,sensitivity,accuracy and AUC of pathological Q wave in the diagnosis of AMI were 63.77%,58.43%,60.76%,and 0.611(0.534-0.688),respectively.The specificity,sensitivity,accuracy and AUC of fragmented QRS wave combined with pathological Q wave in the diagnosis of AMI were 66.67%,74.16%,70.87%,and 0.704(0.632-0.776),respectively.Fragmented QRS wave combined with pathological Q wave has higher AUC in the diagnosis of AMI than fragmented QRS wave and pathological Q wave(Z=5.706,P<0.05;Z=4.251,P<0.05).Conclusions Fragmented QRS wave combined with pathological Q wave in the diagnosis of AMI has higher specificity,sensitivity and AUC,and combined diagnosis has better effect.
