罗哌卡因椎管内麻醉联合喉罩通气全身麻醉在高龄股骨颈骨折患者髋关节置换术中的应用

摘　　要：目的探讨罗哌卡因椎管内麻醉联合喉罩通气全身麻醉在高龄股骨颈骨折患者髋关节置换术中的效果。方法选取2020年10月至2022年5月在我院行髋关节置换术治疗的98例高龄股骨颈骨折患者,随机将其分为两组各49例。对照组实施喉罩通气全身麻醉,观察组实施罗哌卡因椎管内麻醉联合喉罩通气全身麻醉。比较两组的血流动力学指标及疼痛程度。结果两组麻醉诱导前30min、插管时HR、MAP比较差异无统计学意义(P>0.05)。观察组麻醉苏醒后3h、12h、24h的VAS评分低于对照组(P<0.05)。结论罗哌卡因椎管内麻醉联合喉罩通气全身麻醉应用于高龄股骨颈骨折患者髋关节置换术中,未显著影响患者血流动力学指标,但可明显提高镇痛效果,减轻患者麻醉苏醒后的疼痛程度。Objective To explore the effect of ropivacaine spinal anesthesia combined with laryngeal mask ventilation general anesthesia in hip replacement for elderly patients with femoral neck fracture.Methods 98 elderly patients with femoral neck fracture undergoing hip replacement in our hospital from October 2020 to May 2022 were randomly divided into two groups,with 49 cases in each group.The control group was given laryngeal mask ventilation general anesthesia,and the observation group was given ropivacaine spinal anesthesia combined with laryngeal mask ventilation general anesthesia.The hemodynamic indicators and pain degree were compared between the two groups.Results No statistical difference was found in the HR and MAP between the two groups at 30 min before anesthesia induction and intubation(P>0.05).The VAS scores at 3 h,12 h and 24 h after anesthesia recovery of the observation group were lower than those of the control group(P<0.05).Conclusions Ropivacaine spinal anesthesia combined with laryngeal mask ventilation general anesthesia in hip replacement for elderly patients with femoral neck fracture does not significantly affect the hemodynamic indicators,but can significantly improve the analgesic effect,and reduce the pain degree after anesthesia recovery.
