专科个案管理护理模式对原发性醛固酮增多症肾上腺消融术患者的影响

摘　　要：目的探讨专科个案管理护理在原发性醛固酮增多症肾上腺消融术患者中的应用效果。方法选择医院2020年1月-2022年2月收治的原发性醛固酮增多症肾上腺消融术患者112例,按照组间基本特征具有可比性的原则分为对照组与观察组,各56例。对照组采用常规护理模式,观察组采用专科个案模式进行1年的管理,比较两组患者复诊率、护理满意度、降压药服药依从率、血压监测行为依从率。结果观察组患者在接受专科个案管理护理模式干预后3、6、12个月复诊率高于对照组(P<0.05);出院后3个月患者对服务态度、专业技能、疾病宣教、检查治疗安排合理、延续护理满意度评分优于对照组(P<0.05);患者出院1、6个月降压药服药依从率高于对照组(P<0.05);出院3个月血压监测各项行为依从率高于对照组(P<0.05)。结论专科个案管理护理模式能提高原发性醛固酮增多症肾上腺消融术患者的院外复诊率、降压药服用依从率、护理满意度,规范患者院外血压监测行为。Objective To explore the application effect of specialized case management nursing in adrenal ablation patients with primary aldosteronism.Methods 112 patients with primary aldosteronism adrenal ablation admitted to the hospital from January 2020 to February 2022 were selected and divided into a control group and an observation group,each with 56 cases,according to the principle of comparability of basic characteristics between groups.The control group used conventional nursing mode,the observation group used specialist case mode for 1 year's management,comparing the follow-up rate,nursing satisfaction,compliance rate of antihypertensive medication,and compliance rate of blood pressure monitoring behaviour of the two groups of patients.Results The follow-up rate of patients in the observation group was higher than that of the control group at 3,6,and 12 months after the intervention of the specialist case model(P<0.05),the patients'satisfaction scores for service attitude,professional skills,disease education,reasonable arrangement of examination and treatment,and continuity of care were better than those of the control group at 3 months after discharge(P<0.05).The compliance rate of patients'antihypertensive medication taking at 1 and 6 months after discharge was higher than that of the control group(P<0.05),The adherence rate of blood pressure monitoring behaviours was higher than that of the control group at 3 months after discharge(P<0.05).Conclusion Specialist case management nursing model could improve the out-of-hospital follow-up rate,antihypertensive medication compliance rate,nursing satisfaction,and standardise the patients'out-of-hospital blood pressure monitoring behaviours compared with the traditional nursing model in patients with primary aldosteronism adrenal ablation.
