摘　　要：本文就延续护理对COPD患者再入院率的影响进行综述,从延续护理的干预方式、干预内容进行归纳,旨在为建立降低COPD患者再入院的策略和方案提供参考和依据。This article reviewed the effect of transitional care on the readmission rate of chronic obstructive pulmonary disease(COPD)patients,and summarized the intervention methods and contents of transitional care,aiming to provide references and basis for establishing strategies and plans to reduce readmission rate of COPD patients.
