摘　　要：目的制订左心室辅助装置(LVAD)植入患者术后护理推荐意见,为规范LVAD植入患者术后临床护理实践提供参考。方法在全面回顾国内外相关指南、专家共识、系统评价、证据总结等文献基础上,运用循证方法及文献分析法提取LVAD植入患者术后护理推荐建议和研究结论,形成推荐意见初稿,并通过2轮德尔菲专家函询对初稿进行修订和完善,形成正式版推荐意见。结果2轮函询专家积极系数为94%,专家权威系数为0.87,各指标重要性赋值均数均>4.13分,变异系数均<0.18,专家肯德尔和谐系数分别为0.207和0.192(P<0.05)。最终形成包括人员培训、循环系统管理、并发症管理、设备观察及护理、运动康复、健康教育6个部分,共19条推荐意见。结论该推荐意见涵盖LVAD植入术后护理的各环节,基于循证及专家意见基础上形成,具有科学性、严谨性和实用性,可为LAVD植入术后患者的临床护理实践提供指导。Objective To formulate suggestions on postoperative nursing of patients with left ventricular assist device(LVAD)implantation,and to provide references for standardizing postoperative clinical nursing of patients with LVAD implantation.Methods Based on a comprehensive review of domestic and foreign related guidelines,expert consensus,systematic reviews,evidence summary and other literature,evidence-based methods and literature analysis were used to extract postoperative nursing recommendations and research conclusions for LVAD implantation patients,forming a preliminary draft of recommendation opinions.The preliminary draft was revised and improved through two rounds of Delphi expert consultation to form a formal version of recommendation opinions.Results The positivity coefficient of two rounds of correspondence with experts was 94%,and the authority coefficient of experts was 0.87.The average importance assignment of each indicator was greater than 4.13 points,and the coefficient of variation was less than 0.18.The Kendall's harmony coefficients of experts were 0.207 and 0.192,respectively(P<0.05).The final formation included six parts,such as personnel training,circulatory system management,complication management,equipment observation and nursing,exercise rehabilitation and health education,with a total of 19 recommended opinions.Conclusions This recommendation covers all aspects of postoperative nursing for LVAD implantation,which is based on evidence-based and expert opinions.It is scientific,rigorous,and practical,and can provide guidance for clinical nursing practice for patients after LAVD implantation.
