摘 要:目的探索应用末次HIV抗体阴性检测时间模型估计MSM HIV新发感染率适用性。方法2018-2022年每年4-7月,在黑龙江省MSM中开展连续横断面调查,进行HIV抗体检测;应用末次HIV抗体阴性检测时间模型估计黑龙江省MSM HIV新发感染率。采用Logistic回归模型对与末次HIV抗体阴性检测时间模型样本入组相关的影响因素进行分析。结果2018-2022年,共纳入MSM 9864例,其中,3224例样本被纳入末次HIV抗体阴性检测时间模型。模型估计结果显示,2018-2022年黑龙江省MSM HIV新发感染率分别为0.83/100人年(95%CI:0.17/100人年~1.49/100人年),0.62/100人年(95%CI:0.08/100人年~1.16/100人年),0.66/100人年(95%CI:0.01/100人年~1.31/100人年),1.62/100人年(95%CI:0.62/100人年~2.62/100人年),1.53/100人年(95%CI:0.41/100人年~2.65/100人年)。知晓艾滋病相关知识(a OR=2.06,95%CI:1.53~2.76)、最近一年接受过干预服务(a OR=6.12,95%CI:4.27~8.75)等因素为样本纳入末次HIV抗体阴性检测时间模型的独立影响因素。结论在不具备实验室HIV新发感染检测条件或没有条件开展队列调查的地区,末次HIV抗体阴性检测时间模型可以作为替代方法用于HIV新发感染率估计。Objective To explore the applicability of a model of the last negative HIV test time in estimating HIV incidence among men who have sex with men(MSM).Methods Cross-sectional studies were conducted among MSM in Heilongjiang Province every year from April to June between 2018 and 2022,during which HIV-1 antibody testing was performed.A model constructed using the last negative HIV test time was used to estimate HIV incidence.Logistic regression model was performed to identify factors related to the samples included in the model of the last negative HIV test time.Results A total of 9864 MSM were surveyed between 2018 and 2022.Among them,3224 patients were included in the model of the last negative HIV test time.Model estimates of HIV incidence among MSM in Heilongjiang Province from 2018 to 2022 were 0.83/100 person years(PY)(95%CI:0.17/100 PY-1.49/100 PY),0.62/100 PY(95%CI:0.08/100 PY-1.16/100 PY),0.66/100 PY(95%CI:0.01/100 PY-1.31/100 PY),1.62/100 PY(95%CI:0.62/100 PY-2.62/100 PY),and 1.53/100 PY(95%CI:0.41/100 PY-2.65/100 PY).Having knowledge of HIV/AIDS(aOR=2.06,95%CI:1.53-2.76)and having received intervention services in the past year(aOR=6.12,95%CI:4.27-8.75)were independent factors for samples to be included in the model.Conclusions The last negative HIV test time model may be used as an alternative method for estimating HIV incidence in areas where laboratory testing for new HIV infections or cohort surveys are not available.
