盆底超声、血清雌二醇水平与早期产后压力性尿失禁严重程度的相关性分析

摘　　要：目的探究盆底超声、血清雌二醇(E2)水平与早期产后压力性尿失禁(PSUI)严重程度的相关性。方法选取2020年3月~2023年5月于泰州市人民医院分娩的189例产妇。根据产后6~8周是否发生PSUI,将其分为PSUI组(n=103)和非PSUI组(n=86)。比较不同组别中E2、盆底超声相关参数的差异。采用Pearson、Spearman相关性分析评估E2、盆底超声参数与ICI-Q-SF评分的相关性。结果PSUI组血清中E2水平较非PSUI组降低(P<0.05)。PSUI组产妇的V-PUVA、V-LHA、BND及URA均较非PSUI组增大(P<0.05)。随着PSUI严重程度的增加,V-PUVA、V-LHA、BND及URA逐渐升高,而血清E2水平逐渐下降(P<0.05)。Pearson及Spearman相关性分析显示,ICI-Q-SF评分与E2呈负相关,而与V-PUVA、V-LHA、BND及URA呈正相关(P<0.05)。结论血清E2水平可能与PSUI发生有关,对保护盆底组织具有重要意义。盆底超声能够清晰直观地显示盆底的结构和功能。血清E2水平、盆底超声相关参数能够评估PSUI严重程度,能够为临床治疗提供客观依据。Objective To explore the correlation between pelvic floor ultrasound,serum estradiol(E2)level and the severity of early postpartum stress urinary incontinence(PSUI).Methods A total of 189 pregnant women who gave birth in our hospital from March 2020 to May 2023 were selected.The patients were divided into PSUI group(n=103)and non-PSUI group(n=86)according to the occurrence of PSUI at 6 to 8 weeks postpartum.The differ⁃ences of E2 and pelvic floor ultrasound parameters in different groups were compared.Pearson and Spearman cor⁃relation analysis was used to evaluate the correlation between E2,pelvic floor ultrasonic parameters and ICI-QSF score.The predictive value of E2 and pelvic floor ultrasound parameters in PSUI patients was evaluated by re⁃ceiver operator characteristic curve(ROC).Result The serum levels of E2 in PSUI group was lower than that in non-PSUI group(P<0.05).The V-PUVA,V-LHA,BND and URA of women in PSUI group were higher than those in non-PSUI group(P<0.05).With the increase of PSUI severity,V-PUVA,V-LHA,BND and URA gradually increased,while the serum levels of E2 gradually decreased(P<0.05).Pearson and Spearman corre⁃lation analysis showed that ICI-Q-SF score was negatively correlated with E2,but positively correlated with VPUVA,V-LHA,BND and URA(P<0.05).Conclusion The serum levels of E2 may be related to the occurrence of PSUI,which is of great significance for the protection of pelvic floor tissues.Pelvic floor ultrasound can clearly and intuitively display the structure and function of the pelvic floor.The serum levels of E2 and pelvic floor ultra⁃sound related parameters can evaluate the severity of PSUI,which can provide objective basis for clinical treatment.
