摘 要:目的探讨CT成像技术在提高胸部病变肿瘤患者PICC置管定位准确性中的应用效果,以增加一次性置管准确率。方法选取2020年2月至2022年2月在东南大学附属中大医院肿瘤科住院且因治疗需要行PICC置管术的55例存在胸部病变(胸腔积液、纵隔淋巴结转移、左或右肺全切除术后)的肿瘤患者为研究对象。调取患者PICC置管后2周内CT影像计算理想置管长度,分别比较体外预测量置管长度、理想置管长度与实际置管长度的差异。结果55例胸部病变肿瘤患者的预测量置管长度为(38.20±2.32)cm,实际置管长度为(36.11±2.47)cm,两者比较差异有统计学意义(P<0.01),且置管后调管率为100%;根据CT成像计算的理想置管长度为(36.60±2.42)cm,与实际置管长度比较,差异无统计学意义(P>0.01)。结论根据CT成像计算的理想PICC置管长度更接近实际置管长度,对胸部病变肿瘤患者PICC导管尖端定位准确性更高,对于提高患者PICC导管尖端定位准确性及减少相关风险和并发症有积极作用。Objective To explore the role of CT imaging technology in improving the positioning accuracy of PICC placement in cancer patients with chest lesions,so as to increase one-time accuracy rate of PICC placement.Methods A total of 55 cancer patients with chest lesions(pleural effusion,mediastinal lymphatic metastasis,or after left/right pneumonectomy)were selected who were hospitalized in the Oncology Department of Zhongda Hospital Southeast University from February 2020 to February 2022 and required PICC placement.The ideal placement length was calculated by CT imaging within 2 weeks after PICC placement,and the differences between the predicted length in vitro,the ideal length and the actual length were compared respectively.Results For the 55 cancer patients with chest lesions,the predicted placement length was(38.20±2.32)cm,and the actual length was(36.11±2.47)cm,with a statistically significant difference(P<0.01);the post-placement adjustment rate was 100%;the ideal length calculated by CT imaging was(36.60±2.42)cm,and there was no statistically significant difference compared with the actual length(P>0.01).Conclusion The ideal PICC placement length calculated by CT imaging is closer to the actual length,leading to more accurate positioning of PICC tips in cancer patients with chest lesions.Therefore,CT imaging has a positive effect on improving the positioning accuracy of PICC tips and reducing related risks and complications.
