摘　　要：目的:研究尿11脱氢血栓B2(11-DH-TXB2)/尿肌酐在评估PCI围手术期患者抗血小板药物抵抗的临床价值。方法:选取在本院进行PCI术的急性冠脉综合征患者231名为研究对象,根据血小板聚集功能实验,将其分为抵抗组(n=48)、半抵抗组(n=112)和有效组(n=71)。分析三组间尿11-DH-TXB2和尿11-DH-TXB2/尿肌酐的水平。评价尿11-DH-TXB2/尿肌酐在在评估PCI围手术期患者抗血小板药物抵抗的临床价值。结果:与有效组相比,半抵抗组和抵抗组中尿11-DH-TXB2和尿11-DH-TXB2/尿肌酐的水平升高,抵抗组较半抵抗组升高更明显(P<0.05),差异具有显著的统计学意义。Pearson相关分析结果显示,尿11-DH-TXB2/尿肌酐比值与AA最大聚集率和ADP最大聚集率均成显著正相关(r=0.491、0.249,P<0.05)。采用ROC曲线评估尿11-DH-TXB2/尿肌酐对血小板药物抵抗的临床价值,结果显示当尿11-DH-TXB2/尿肌酐比值为2215.76时,其灵敏度为0.909,特异度为0.775,曲线下面积(AUC)=0.895。结论:尿11-DH-TXB2/尿肌酐的水平可用于评估PCI围手术期患者抗血小板药物抵抗。Objective:To investigate the clinical value of urinary 11 dehydrothrombotic B2/creatinine in evaluating antiplatelet drug resistance in perioperative patients undergoing PCI.Method:231 patients with acute coronary syndrome who underwent PCI in our hospital were selected as the research subjects.Based on platelet aggregation function experiments,they were divided into resistance group(n=48),semi resistance group(n=112),and effective group(n=71).The levels of urine 11-dehydrothrombin B2 and urine 11-dehydrothrombin B2/creatinine among the three groups were analyzed.The clinical value of urine 11-dehydrothrombin B2/creatinine in evaluating antiplatelet drug resistance in patients undergoing PCI perioperative period was evaluated.Results:Compared with the effective group,the levels of urine 11-dehydrothrombotane B2 and urine 11-dehydrothrombotane B2/creatinine in the semi resistant and resistant groups were significantly increased,the resistance group showed a more significant increased than the semi resistance group(P<0.05).The Pearson correlation analysis results showed that there was a significant positive correlation between the ratio of urine 11-dehydrothrombin B2/creatinine and the maximum aggregation rate of AA and ADP,with r values of 0.491 and 0.249(P<0.05).ROC curve results showed that when the ratio of urinary 11-dehydrothrombotane B2/creatinine was 2215.76,the sensitivity was 0.909,the specificity was 0.775,and the area under the curve(AUC)was 0.895.Conclusion:Urine 11 dehydrothrombotic B2/creatinine can be used to evaluate antiplatelet drug resistance in patients undergoing PCI perioperative period.
