退翳明目汤辅助治疗翼状胬肉风热壅盛证41例临床观察

摘　　要：目的 观察退翳明目汤辅助治疗翼状胬肉风热壅盛证的临床疗效。方法 将82例翼状胬肉风热壅盛证患者采用随机数字表法分为对照组和治疗组,各41例。对照组采用翼状胬肉切除术联合角膜缘干细胞移植术治疗,治疗组在对照组治疗方法的基础上给予退翳明目汤治疗。治疗30 d后比较2组治疗前后泪膜破裂时间(BUT)及泪液分泌试验(SIT)值;治疗后随访6个月,比较2组的复发率。结果 治疗后2组BUT明显延长,SIT值明显升高,与同组治疗前比较差异均有统计学意义(P<0.05或P<0.01),且治疗组改善更明显,与对照组治疗后比较差异均有统计学意义(P<0.05或P<0.01);随访6个月后,治疗组复发率为2.4%(1/41),对照组为19.5%(8/41),2组比较差异有统计学意义(P<0.05)。结论 退翳明目汤辅助治疗翼状胬肉风热壅盛证临床疗效显著,可有效增强患者泪膜稳定性,改善泪液分泌量,降低复发率,值得临床推广应用。Objective To observe the clinical efficacy of Tuiyi Mingmu Tang(退翳明目汤)as an auxiliary treatment for the syndrome of wind heat stagnation in pterygium.Methods 82 patients with wind heat syndrome of pterygium were randomly divided into a control group and a treatment group by random number table method,with 41 cases in each group.The control group was treated with pterygium excision combined with corneal limbal stem cell transplantation,while the treatment group was treated with Tuiyi Mingmu Tang on the basis of the control group’s treatment method.After 30 days of treatment,the tear film break-up time(BUT)and schirmer test(SIT)values were compared between the two groups before and after treatment.Follow up for 6 months after treatment and compare the recurrence rates between the two groups.Results After treatment,the BUT significantly prolonged and SIT values significantly increased in both groups,with a statistically significant difference compared to the same group before treatment(P<0.05 or P<0.01),and the improvement was more significant in the treatment group,with a statistically significant difference compared to the control group after treatment(P<0.05 or P<0.01).After a 6-month follow-up,the recurrence rate in the treatment group was 2.4%(1/41),while in the control group it was 19.5%(8/41).The difference between the two groups was statistically significant(P<0.05).Conclusion Tuiyi Mingmu Tang has a significant clinical effect in the auxiliary treatment of pterygium with wind heat stagnation syn-drome.It can effectively enhance the stability of the patient’s tear film,improve tear secretion,and reduce recur-rence rate,which is worthy of clinical promotion and application.
