6F尿管作为封堵器在经尿道钬激光碎石术中的应用

摘　　要：目的探讨将6F尿管作为封堵器应用于经尿道钬激光碎石术中的可行性和安全性。方法回顾性分析本院2019年12月至2020年3月收治的行经尿道钬激光碎石术的35例输尿管下段结石患者的临床资料,将其中17例未使用封堵器的患者设为对照组,另外18例应用6F尿管作为结石封堵器的患者设为观察组,比较分析两组的一般资料和治疗效果。结果观察组的结石逆行移位少于对照组,差异有统计学意义(P=0.002);两组的合并肾或输尿管积水、结石逆入肾脏改用软镜、输尿管破损、术后24 h结石残留、术后24 h炎症反应综合征(SIRS)发生情况比较,差异均无统计学意义(均P>0.05)。结论6F尿管经简单改造后可用于中下段输尿管结石钬激光手术中,其明显减少了术中结石移位现象,安全可行且经济适用。Objective To investigate the feasibility and safety of using 6F urinary tube as a stopper in transurethral holmium laser lithotripsy.Methods The clinical data of 35 patients with lower ureteral calculi who underwent transethral holmium laser lithotripy admitted to our hospital from December 2019 to March 2020 were retrospectively analyzed.Among them,17 patients did not use the occluder as the control group,and 18 patients used the 6F urinary tube as the stone occluder as the observation group.General data and clinical outcomes were compared between the two groups.Results The retrograde displacement of stones in observation group was less than that in control group,and the difference was statistically significant(P=0.002).There were no significant differences in renal or ureteral hydronephrosis,calculus retroentry to kidney using soft lens,ureter damage,stone residue 24 h after surgery,and SIRS 24 h after surgery between the two groups(all P>0.05).Conclusions 6F urinary tube can be used in holmium laser surgery for middle and lower ureteral calculi after simple modification,which significantly reduces intraoperative stone displacement,is safe,feasible and economical.
