摘  要:目的探讨血管紧张素Ⅱ1型受体[AGTR1(A1166C)]基因多态性与中国汉族人群糖尿病肾病(DN)患者微血管病变易感性的相关性。方法选取2020年1月至2021年3月沧州市人民医院收治的100例2型糖尿病患者, 根据尿白蛋白/肌酐比值(UACR)将其分为糖尿病非肾病组(40例)和DN组(60例)。同期另选取于本院体检的健康人群50例作为对照组。将DN组分为C等位基因携带[AGTR1(A1166C)基因的等位基因为C, 10例]和非C等位基因携带组[AGTR1(A1166C)基因的等位基因为A, 50例]以及微量白蛋白尿组(30 mg/g≤UACR<300 mg/g, 40例)和大量白蛋白尿组(UACR≥300 mg/g, 20例)。采用实时荧光定量PCR法检测两组受检者的AGTR1(A1166C)基因多态性;非条件logistic回归模型分析AGTR1(A1166C)基因多态性与DN发生风险的相关性。结果 DN组和糖尿病非肾病组的收缩压(SBP)、舒张压(DBP)、血清肌酐(Scr)、尿素氮(BUN)、空腹血糖(FPG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C), 糖化血红蛋白(HbA1c)水平及UACR高于对照组, 三酰甘油(TG)低于对照组, 且DN组高于糖尿病非肾病组(均P<0.05)。Hardy-Weinberg平衡分析发现, 对照组、DN组和糖尿病非肾病组人群的AA、AC、CC三种基因型分布均符合基因遗传平衡法则(均P>0.05)。DN组的C等位基因分布频率高于糖尿病非肾病组、对照组(均P<0.05), DN组、糖尿病非肾病组的AC、CC基因频率高于对照组, 且DN组高于糖尿病非肾病组(均P<0.05)。非条件logistic回归模型分析结果显示, C等位基因型携带者发生DN的风险高于A等位基因携带者(OR=1.583, 95%CI:1.094~2.072, P<0.05);AC+CC基因型携带者发生DN的风险高于AA基因型携带者(OR=1.924, 95%CI:1.437~2.411, P<0.05)。C等位基因携带组的SBP、DBP、BUN、Scr、FPG、LDL-C、HDL-C、HbA1c水平及UACR高于非C等位基因携带组, 差异均有统计学意义(均P<0.05)。结论 AGTR1(A1166C)基因多态性与中国汉族人群DN发Objective To investigate the correlation between angiotensin II type 1 receptor gene AGTR1(A1166C)polymorphism and microangiopathopathy susceptibility in diabetic nephropathy(DN)patients in Chinese Han population.Methods A total of 100 patients with type 2 diabetes admitted to Cangzhou People's Hospital from January 2020 to March 2021 were selected and divided into diabetic non-nephrotic group(40 cases)and DN group(60 cases)according to urinary albumin/creatinine ratio(UACR).In the same period,50 healthy people who underwent physical examination in our hospital were selected as the control group.The DN group was divided into C allele carrier group[AGTR1(A1166C)gene allele C,10 cases],non-C allele carrier group[AGTR1(A1166C)gene allele A,50 cases]and microalbuminuria group(30 mg/g≤UACR<300 mg/g,40 cases)and macroalbuminuria group(UACR≥300 mg/g,20 cases).The ACTR1(A1166C)gene polymorphisms were detected by real-time fluorescence quantitative PCR.The correlation between AGTR1(A1166C)gene polymorphisms and the risk of DN was analyzed by unconditional logistic regression model.Results Systolic blood pressure(SBP),diastolic blood pressure(DBP),serum creatinine(Scr),urea nitrogen(BUN),fasting blood glucose(FPG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)in DN group and diabetic non-nephrotic group.The level of HbAlc and UACR were higher than that of control group,and triglyceride(TG)was lower than that of control group,and the DN group was higher than that of diabetic non-nephrotic group(all P<0.05).Hardy-Weinberg balance analysis showed that the distribution of AA,AC and CC genotypes in control group,DN group and diabetic non-nephrotic group were consistent with the rule of genetic balance(all P>0.05).The distribution frequency of C allele in DN group was higher than that in diabetic non-nephrotic group and control group(all P<0.05),the frequency of AC and CC gene in DN group and diabetic non-nephrotic group was higher than that in control group,and DN group was higher tha
