摘　　要：目的:总结4例新型冠状病毒感染危重型患者(以下简称新冠病毒感染危重型患者)成功救治的临床特征和诊疗经验。方法:回顾分析贵州中医药大学第二附属医院综合救治的4例新冠病毒感染危重型患者的临床资料和治疗经过。结果:4例患者均因“呼吸困难”入院,核酸抗原示阳性。积极予气管插管、抗病毒、充分镇静镇痛、俯卧位通气、辨证使用中药汤剂胃注、抗感染、升压、抗凝、营养支持、调节电解质紊乱等对症治疗,患者临床症状缓解,脱机成功。恢复情况良好。结论:新型冠状病毒致病力强,危重型患者死亡率高,总结4例成功救治患者经验,表明新冠病毒感染危重型患者采取积极气管插管、充分镇静镇痛、俯卧位通气、中西医结合辨证施治等综合治疗措施有助于患者的救治。Objective:To summarize the clinical characteristics and diagnosis and treatment experience of four critically ill patients with COVID-19.Methods:The clinical data and treatment course of 4 critically ill patients with SARS-CoV-2 infection who were treated in the Second Affiliated Hospital of Guizhou University of Traditional Chinese Medicine were retrospectively analyzed.Results:Four patients were admitted due to dyspnea,and the nucleic acid antigen was positive back.The patient was actively treated with endotracheal intubation,antiviral therapy,sufficient sedation and analgesia,prone position ventilation,dialectical gastric injection of traditional Chinese medicine decoction,anti-infection therapy,hypertension therapy,anticoagulation therapy,nutritional support,and regulation of electrolyte disorder.The clinical symptoms of the patient were relieved and the ventilator was successfully weaned.The recovery is good.Conclusion:SARS-CoV-2 has strong pathogenicity,and the mortality rate of critical patients is high.To summarize the four successfully treated patients,it is concluded that the comprehensive treatment of critical patients with SARS-CoV-2 infection,such as active tracheal intubation,adequate sedation and analgesia,prone position ventilation,and dialectic treatment of integrated traditional Chinese and western medicine,is helpful to the successful treatment of patients.
