2型糖尿病合并高脂血症气阴两虚证及肝肾阴虚证舌象客观化研究

摘　　要：目的:通过分析2型糖尿病合并高脂血症气阴两虚证及肝肾阴虚证患者舌象特征参数,为两种证型的临床辨证分型提供实验依据。方法:用TFDA-1型舌面诊仪采集2型糖尿病合并高脂血症气阴两虚证患者253例、肝肾阴虚证患者74例及健康人130名舌象图片,用TDAS2.0软件进行舌象参数提取与分析。结果:与健康组比较,气阴两虚证组舌质颜色特征参数Lb显著升高(P<0.01),G、B、H、I、L、Y、Cb显著降低(P<0.05,P<0.01),舌苔颜色特征参数S、Lb显著升高(P<0.01),G、B、H、I、L、Y、Cb显著降低(P<0.01),舌苔厚薄特征参数perAll、perPart显著降低(P<0.05);肝肾阴虚证组舌质颜色特征参数La、Lb、Cr显著升高(P<0.01),G、B、H、I、L、Y、Cb显著降低(P<0.01,P<0.05),舌苔颜色特征参数S、Lb、Cr显著升高(P<0.01,P<0.05),G、B、H、I、L、Y、Cb显著降低(P<0.01,P<0.05),舌苔厚薄特征参数perAll、perPart显著降低(P<0.01,P<0.05)。与气阴两虚证组比较,肝肾阴虚证组舌质颜色特征参数La、Cr显著升高(P<0.01,P<0.05);舌苔颜色特征参数La显著升高(P<0.01)。结论:舌象特征参数提示气阴两虚证组患者舌象呈舌质淡暗,舌苔少而黄的表现;肝肾阴虚证组患者舌象呈舌体暗无光泽,舌质红,舌苔少而黄的表现;与中医舌诊观察结果基本符合。Objective:To provide experimental basis for the clinical syndrome differentiation of type 2 diabetes mellitus(T2DM)combined with hyperlipidemia by analyzing the characteristic parameters of tongue appearance in patients with qi and yin deficiency syndrome and liver and kidney yin deficiency syndrome.Methods:The tongue images of 253 patients with T2DM complicated with hyperlipidemia with qi and yin deficiency,74 patients with liver and kidney yin deficiency,and 130 healthy subjects were collected by TFDA-1 tongue face diagnostic instrument.The tongue images parameters were extracted and analyzed by TDAS 2.0 software.Results:Compared with the healthy group,Lb was significantly increased(P<0.01),G,B,H,I,L,Y,Cb were significantly decreased(P<0.05,P<0.01),S and Lb were significantly increased(P<0.01)in the qi and yin deficiency syndrome group.G,B,H,I,L,Y,Cb were significantly decreased(P<0.01),and the characteristic parameters of tongue coating thickness perAll and perPart were significantly decreased(P<0.05).In the yin deficiency of liver and kidney syndrome group,the tongue color characteristic parameters La,Lb and Cr were significantly increased(P<0.01),G,B,H,I,L,Y and Cb were significantly decreased(P<0.01,P<0.05),and the tongue coating color characteristic parameters S,Lb and Cr were significantly increased(P<0.01,P<0.05).G,B,H,I,Y and Cb were significantly decreased(P<0.01,P<0.05),and the characteristic parameters perAll and perPart of tongue coating thickness were significantly decreased(P<0.01,P<0.05).Compared with the qi and yin deficiency syndrome group,the tongue color characteristic parameters La and Cr in the liver and kidney yin deficiency syndrome group were significantly increased(P<0.01,P<0.05).The color characteristic parameter La of tongue coating was significantly increased(P<0.01).Conclusion:The characteristic parameters of tongue image suggest that the tongue of the patients with qi and yin deficiency syndrome is light and dark,with less and yellow tongue coating.In the group of yin deficiency of
