三黄煎剂治疗三阴性乳腺癌气虚血瘀证的临床观察

摘　　要：目的:观察三黄煎剂对三阴性乳腺癌气虚血瘀证的临床疗效。方法:收集100例三阴性乳腺癌患者,治疗予以三黄煎剂口服,服用半年,观察两年。观察患者两年无病生存率以及应激症状积分、生活质量积分、炎症因子(IL-6、IL-8)水平、血管新生因子(VEGF)水平、血清氧化应激指标(NO、GST、SOD)。根据两年无病生存率结果继续选取符合标准的三阴性乳腺癌患者40例进行随机、平行、对照研究,分为对照组和治疗组,每组20例。治疗组予三黄煎剂口服,服用半年,对照组定期观察。观察患者的气虚证、血瘀证积分情况,生活质量积分情况,肿瘤标志物(CEA、CA-153)水平、炎症因子(IL-4、IL-8、IL-2、IFN-γ)水平、免疫因子(CD3、CD4、CD8)水平、血清氧化应激指标(SOD、GSH-Px)。结果:100例三阴性乳腺癌患者脱落13例,87例患者进行分析。两年无病生存率为100%。IL-6、IL-8及NO、GST、SOD治疗后得到显著改善(P<0.01,P<0.05),患者症状积分、生活质量积分均较治疗前显著改善(P<0.01),安全性指标均在合理范围内,未见异常。随机、平行、对照研究中治疗组脱落4例,对照组脱落5例。与对照组比较,治疗组气虚证、血瘀证积分显著改善(P<0.01),生命质量在疲倦、疼痛、气促、食欲丧失、腹泻方面得到显著改善(P<0.05),总体积分情况也得到显著改善(P<0.05),CEA、CA-153治疗后均下降,CA-153显著改善(P<0.05),IL-2、IL-4、IL-8、IFN-γ、GSH-Px、SOD治疗后得到显著改善(P<0.05),安全性指标均在合理范围内,未见异常。结论:三黄煎剂可以改善患者炎症因子水平、慢性氧化应激状态,提高患者免疫功能,改善患者的气虚血瘀症状,提高患者生命质量,具有良好疗效。Objective:To observe the clinical efficacy of Sanhuang Decoction in the treatment of triple negative breast cancer with qi deficiency and blood stasis syndromes.Methods:A total of 100 cases of triple negative breast cancer patients were collected and treated with Sanhuang Decoction for half a year and observed for two years.The two-year disease-free survival rate of patients,as well as their stress symptom scores,quality of life scores,inflammatory factor levels(IL-6,IL-8),angiogenesis factor levels(VEGF),and serum oxidative stress(NO,GST,SOD)were observed.Based on the two-year disease-free survival rate results,40 triple negative breast patients who met the standards were further selected for a randomized,parallel,and controlled study.The patients were divided into a control group and a treatment group,with 20 cases in each group.The treatment group received Sanhuang Decoction orally for six months,while the control group was regularly observed.The patient's score of qi deficiency syndrome and blood stasis syndrome,as well as the improvement of quality of life score,tumor marker levels(CEA,CA-153),inflammatory factor levels(IL-4,IL-8,IL-2,IFN-γ),immune factor levels(CD3,CD4,CD8),and serum oxidative stress indicators(SOD,GSH-Px)were observed.Results:Among 100 cases of triple negative breast cancer,13 cases dropped out,and 87 cases were analyzed.The two-year disease-free survival rate was 100%.There was a significant improvement in these indexs:IL-6,IL-8,NO,GST,and SOD after treatment(P<0.01,P<0.05).The patients'symptom score and quality of life score were significantly improved compared to before(P<0.01),and the safety indicators were within a reasonable range,without any abnormalities.In a randomized,parallel,and controlled study,there were 4 cases of dropout in the treatment group and 5 cases of dropout in the control group.Compared with the control group,there was a significant improvement in the treatment group in the scores of qi deficiency syndrome and blood stasis syndrome(P<0.01),and the quality of life
