血府逐瘀汤治疗胃食管反流病气滞血瘀证的临床疗效及其对自主神经功能的影响

摘　　要：目的:观察血府逐瘀汤治疗胃食管反流病(GERD)气滞血瘀证患者的临床疗效及其对自主神经功能的影响。方法:选取GERD气滞血瘀证患者58例,随机分为对照组(28例)和治疗组(30例)。对照组患者给予糊精联合艾司奥美拉唑镁肠溶片,治疗组患者给予血府逐瘀汤联合艾司奥美拉唑镁肠溶片,疗程均为8周。治疗前后使用胃镜下分级和反流性疾病问卷(RDQ)积分评估GERD食管症状的程度;使用气滞血瘀证量表(QSBSSS)积分评估气滞血瘀证的程度;使用自主神经症状评价量表(ASP)积分评估主神经紊乱的程度。比较两组患者胃镜下分级、食管症状、气滞血瘀证和自主神经紊乱的改善有效率。结果:治疗组患者胃镜下分级、RDQ积分、QSBSSS积分和ASP积分改善有效率均显著优于对照组(P<0.05)。结论:血府逐瘀汤不仅能够改善GERD气滞血瘀证患者的胃镜下炎症程度、食管症状和中医证候,还能有效纠正自主神经紊乱。Objective:To investigate the clinical effect of Xuefu Zhuyu Decoction in treating of gastroesophageal reflux disease(GERD)with qi stagnation and blood stasis syndrome and the impact on autonomic nerve function.Methods:A total of 58 patients with GERD with qi stagnation and blood stasis syndrome were randomly divided into two groups:control group(28 cases)and treatment group(30 cases).Patients in the control group were given dextrin and esomeprazole enteric-coated tablets,and patients in the treatment group were given Xuefu Zhuyu Decoction and esomeprazole enteric-coated tablets.The treatments lasted for 8 weeks.Before and after treatment,the two groups were scored using the endoscopic grading and Reflux Diagnostic Questionnaire(RDQ)to evaluate the degree of esophageal symptoms,Qi Stagnation and Blood Stasis Syndrome Scale(QSBSSS)to evaluate traditional Chinese medicine(TCM)syndromes,and Autonomic Symptom Profile(ASP)to evaluate the degree of autonomic dysfunction.The effective rates on the esophageal symptoms,qi stagnation and blood stasis syndrome,and autonomic dysfunction were compared between two groups.Results:The effective rates on the endoscopic grading,RDQ,QSBSSS and ASP in the treatment group were superior to the control group(P<0.05).Conclusion:Xuefu Zhuyu Decoction can improve the endoscopic grading,the esophageal symptom,TCM syndromes and autonomic dysfunction in patients with GERD with qi stagnation and blood stasis syndrome,and it can also effectively correct autonomic dysfunction.
