如意金黄散序贯外治对肛周脓肿术后创面及细菌清除率的影响

摘　　要：目的:观察如意金黄散序贯外治对肛周脓肿术后创面及细菌清除率的影响。方法:选取肛周脓肿术后患者116例,采用随机数字表法分为观察组和对照组各58例。对照组给予高锰酸钾熏洗联合凡士林纱条填塞治疗,观察组给予如意金黄散序贯外治治疗。比较2组疼痛评分、肉芽组织评分、炎症因子水平、细菌清除率、创面愈合时间,评价2组临床疗效。结果:观察组总有效率为89.66%,高于对照组68.97%(P<0.05)。治疗3 d、8 d、14 d后,观察组疼痛评分和肉芽组织生长评分均低于对照组(P<0.05)。治疗后,2组C-反应蛋白(CRP)、白细胞介素-6 (IL-6)水平较治疗前降低(P<0.05),且观察组CRP、IL-6水平低于对照组(P<0.05)。治疗第3天,观察组细菌清除率为31.03%,高于对照组8.62%(P<0.05)。治疗第8天,观察组细菌清除率为91.38%,高于对照组74.14%,(P<0.05)。结论:如意金黄散序贯外治运用于肛周脓肿术后可减轻局部疼痛,加快肉芽组织生长,减轻炎症因子水平,加快细菌清除,缩短创面愈合时间,提高临床疗效,极大地减轻了患者的痛苦。Objective:To observe the effect of sequential external treatment of Ruyi Jinhuang Powder on wound and bacteria clearance rates after operation for perianal abscess.Methods:A total of 116 cases of patients after operation for perianal abscess were selected and divided into the observation group and the control group according to the random number table method,with 58 cases in each group.The control group was given fumigation with potassium permanganate and filled with vaseline gauze,and the observation group was given sequential external treatment of Ruyi Jinhuang Powder.The pain scores,scores of granulation tissue,levels of inflammatory factors,bacteria clearance rates,wound healing time were compared between the two groups.The clinical effects in the two groups were evaluated.Results:The total effective rate was 89.66% in the observation group,higher than that of 68.97% in the control group(P<0.05).After treatment for 3,8 and 14 days,the pain scores and granulation tissue growth scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of C-reactive protein(CRP) and interleukin 6(IL-6) in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of CRP and IL-6 in the observation group were lower than those in the control group(P<0.05).On the third day of treatment,the bacterial clearance rate was 31.03% in the observation group,higher than that of 8.62% in the control group(P<0.05).On the eighth day of treatment,the bacterial clearance rate was 91.38% in the observation group,higher than that of 74.14% in the control group(P<0.05).Conclusion:Sequential external treatment of Ruyi Jinhuang Powder can alleviate local pain,accelerate granulation tissue growth,reduce levels of inflammatory factors,accelerate bacterial clearance,shorten wound healing time,improve clinical treatment,and greatly alleviate patient pain after operation for perianal abscess.
