复方血栓通胶囊联合雷珠单抗治疗视网膜静脉阻塞伴黄斑水肿临床研究  

Clinical Study on Compound Xueshuantong Capsules Combined with Lucentis for Retinal Vein Obstruction Complicated with Macular Edema

摘　　要：目的:观察复方血栓通胶囊联合雷珠单抗治疗视网膜静脉阻塞(RVO)伴黄斑水肿的临床疗效及对血液流变学的影响。方法:80例RVO伴黄斑水肿患者按随机数字表法分为对照组、观察组各40例。对照组给予玻璃体腔注射雷珠单抗治疗,观察组在对照组基础上联用复方血栓通胶囊治疗。比较2组治疗前后中医证候积分、黄斑中心凹厚度、最佳矫正视力(BCVA)、血液流变学指标,并评估2组临床疗效及不良反应发生情况。结果:观察组总有效率高于对照组(P<0.05)。2组治疗后中医证候积分、黄斑中心凹厚度、纤维蛋白原、血浆黏度、红细胞聚集指数均较治疗前降低(P<0.05),BCVA升高(P<0.05),且观察组治疗后中医证候积分、黄斑中心凹厚度、血液流变学指标低于对照组(P<0.05),BCVA高于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:复方血栓通胶囊联合雷珠单抗治疗RVO伴黄斑水肿疗效确切,能够缓解患者黄斑水肿症状,改善血液流变学及视力,安全性较高。Objective:To observe the clinical effect of Compound Xueshuantong Capsules combined with Lucentis for retinal vein obstruction(RVO) complicated with macular edema and its effect on hemorheology.Methods:A total of 80 cases of patients with RVO complicated with macular edema were divided into the control group and the observation group according to the random number table method,with 40 cases in each group.The control group was treated with intravitreal injection with Lucentis,and the observation group was additionally treated with Compound Xueshuantong Capsules based on the treatment of the control group.Before and after treatment,traditional Chinese medicine(TCM) syndrome scores,thickness of macular fovea,Best Corrected Visual Acuity(BCVA),and hemorheology indexes were compared between the two groups.The clinical effects and the occurrence of adverse reactions in the two groups were evaluated.Results:The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,TCM syndrome scores,thickness of macular fovea,fibrinogen,plasma viscosity,and red blood cell aggregation indexes in the two groups were decreased when compared with those before treatment(P<0.05),and BCVA was increased(P<0.05);TCM syndrome scores,thickness of macular fovea,and hemorheology indexes in the observation group were lower than those in the control group(P<0.05),and BCVA was higher than that in the control group(P<0.05).There was no significant difference being found in the comparison of the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compound Xueshuantong Capsules combined with Lucentis has a definite curative effect in the treatment of RVO complicated with macular edema,which can alleviate the symptoms of macular edema and improve hemorheology and visual acuity,with high safety.
