摘　　要：目的 通过Meta分析的方法探讨脓毒性休克患者应用参附注射液对其早期血流动力学的影响。方法 通过检索中国知网(CNKI)、维普中文科技期刊(VIP)、中国生物医学文献数据库(CBM)、万方数据(WanFang)、PubMed、The Cochrane Library等数据库中关于参附注射液治疗脓毒性休克的随机对照研究,检索时间限定为自建库以来至2023年1月31日。使用Review Manger5.3软件对所提取的数据进行Meta分析。结果 在纳入的9篇研究中,治疗24 h后观察组心指数(CI)、体循环血管阻力指数(SVRI)、平均动脉压(MAP)均明显高于对照组,血管外肺水指数(EVLWI)、血乳酸(Lac)、中心静脉压(CVP)均明显低于对照组[MD=0.30,95%CI(0.27,0.33),P<0.000 01;MD=63.36,95%CI(3.77,122.94),P=0.04;MD=2.52,95%CI(0.99,4.06),P=0.001;MD=-0.52,95%CI(-0.94,-0.10),P=0.02;MD=-0.81,95%CI(-1.31,-0.31),P=0.001;MD=-7.20,95%CI(-10.34,-4.07),P=0.001]。结论 常规治疗联合参附注射液能更好地改善脓毒性休克患者的早期血流动力学状态。Objective:To exploring the effect of Shenfu Injection on early hemodynamics in patients with septic shock through meta-analysis.Methods:Randomized controlled trials(RCTs)on Shenfu Injection in the treatment of septic shock were searched in databases of CNKI,VIP,CBM,WanFang,PubMed and The Cochrane Library from inception to January 31,2023.Meta analysis of the extracted data was performed using Review Manger5.3software.Results:In the 9 included studies,the cardiac index(CI),systemic vascular resistance index(SVRI)and mean arterial pressure(MAP)24h after treatment in the observation group were significantly higher than those in the control group,and the extravascular pulmonary water index(EVLWI),blood lactic acid(Lac)and central venous pressure(CVP)were significantly lower than those in the control group[MD = 0.30,95%CI(0.27,0.33),P<0.000 01;MD = 63.36,95%CI(3.77,122.94),P = 0.04;MD = 2.52,95%CI(0.99,4.06),P = 0.001;MD =-0.52,95%CI(-0.94,-0.10),P = 0.02;MD =-0.81,95%CI(-1.31,-0.31),P = 0.001;MD =-7.20,95%CI(-10.34,-4.07),P =0.001].Conclusion:Shenfu Injection can effectively improve the early hemodynamics of septic shock patients.
