摘  要:目的观察炎调方调节肺泡上皮细胞钠水转运系统,维持肺泡内外液体平衡的作用和机制。方法体内实验:雄性成年SD大鼠分为模型组、炎调方组、正常对照组和假手术组,盲肠结扎穿孔术(CLP)制备脓毒症模型。分别于CLP后2、8、12、24、48 h麻醉腹主动脉采血处死并留取标本进行血气分析、血清SP-D水平、肺湿重/干重(W/D)、钠水通道相关蛋白检测等。体外实验:检测炎调方药物血清对大鼠肺泡Ⅱ型细胞存活率、上清液SP-D水平、钠水通道相关蛋白表达等的影响。结果炎调方中、高剂量组均降低了脓毒症大鼠肺湿重/干重(P<0.05)。炎调方中、高剂量组均升高了脓毒症大鼠PO2(P<0.05)。炎调方低、中、高剂量组均能显著抑制模型组诱导的SP-D水平(P<0.05)。炎调方低、中、高剂量组增加钠水转运系统蛋白表达。炎调方中剂量和高剂量处理细胞12 h均显著降低了LPS诱导的SP-D水平(P<0.05)。炎调方处理细胞2 h和12 h,中、高剂量均能增加钠水转运系统蛋白表达。结论炎调方能通过调节大鼠肺泡Ⅱ型上皮细胞调控钠水转运系统,缓解脓毒症大鼠肺水肿。Objective:To observe the effect of Yantiao Decoction on regulating the sodium-water transport system of alveolar epithelial cells and maintaining the fluid balance inside and outside the alveoli.Methods:For in vivo experiments,SD adult male rats were divided into normal control,sham-operated,model and inflammatory conditioning groups,and sepsis models were prepared by cecum ligation perforation(CLP).Blood was collected from the anesthetized abdominal aorta at 2,8,12,24 and 48 h after CLP,and specimens were collected for blood gas analysis,measure lung wet and dry weight ratio,sodium-water channel-related protein,and serum SP-D level.In vitro experiments were conducted to detect the effects of Yantiao Decoction drug serum on the survival rate,supernatant SP-D level and channel-related protein expression of rat alveolar typeⅡcells.Results:Both the medium and high dose groups of Yantiao Decoction reduced the dry and wet weight specific gravity of lung in septic rats(P<0.05).The PO2 of septic rats was increased in both the medium and high dose groups of Yantiao Decoction(P<0.05).The low,medium and high dose groups of Yantiao Decoction significantly inhibited the level of SP-D induced by the model group(P<0.05).The expression of sodium-water transport system protein was increased in the low,medium and high dose groups of Yantiao Decoction.Both medium and high doses of Yantiao Decoction treated cells for 12 h significantly reduced the LPS-induced SP-D levels(P<0.05).Yantiao Decoction treated cells for 2 h and 12 h,and both medium and high doses increased the protein expression of sodium-water transport system.Conclusion:Yantiao Decoction can regulate the sodium-water transport system by modulating the alveolar typeⅡepithelial cells of rats and alleviate pulmonary edema in septic rats.
