早期经穴推拿对ICU重症肺炎机械通气患者获得性衰弱的影响

摘　　要：目的 观察早期经穴推拿对重症肺炎机械通气患者ICU获得性衰弱(ICU-AW)的影响。方法 将60例ICU-AW患者随机分为治疗组与对照组各30例。两组均给予常规治疗与护理,治疗组在此基础上予以经穴推拿,每天1次,每次40 min,每周5次。比较两组患者英国医学研究理事会(MRC)评分、ICU-AW的发生率、股直肌横截面积、巴氏评分(BI评分)、机械通气时间、ICU住院时间。结果 干预5 d后治疗组MRC评分、BI评分高于对照组,ICU-AW发生率低于对照组,股直肌横截面积大于对照组,机械通气时间和ICU住院时间均低于对照组(P<0.05)。结论 早期经穴推拿对ICU重症肺炎机械通气患者肌力有改善作用,同时能改善患者日常生活能力。Objective:To explore the effect of early acupoint massage on intensive care unit acquired weakness(ICU-AW)in patients with severe pneumonia undergoing mechanical ventilation.Methods:A total of 60 patients were randomly divided into the intervention group and the control group,with 30 cases in each group.Both groups were given routine treatment and nursing,and the observation group was given acupoint massage on the basis of routine treatment and nursing,40 minutes each time,once a day,5 times a week.The British Medical Research Council(MRC)score,the incidence of ICU-AW,quadriceps femoris cross-sectional area,Bus Index score(BI score),mechanical ventilation time,and ICU length of stay were compared between the two groups.Results:After 5 days of intervention,the MRC score,BI score of the intervention group were higher than of the control group.The incidence of ICU-AW in the study group was lower than that in the control group.The crosssectional area of quadriceps femoris of the intervention group was higher than of the control group.The duration of mechanical ventilation and ICU stay in the intervention group were lower than those in the control group(P<0.05).Conclusion:Early acupoint massage can improve muscle strength and activity of daily living in ICU pa⁃tients with severe pneumonia undergoing mechanical ventilation.
