平脉定悸方对快速型心律失常患者血清IL-1β、NF-κB及心率变异性的影响

摘　　要：目的 探讨平脉定悸方加减治疗快速型心律失常的疗效及对血清白细胞介素-1β(IL-1β)、核因子-κB(NF-κB)和心率变异性的影响。方法 选取78例重度快速型心律失常急发患者,按照随机数字表法分为两组各39例,经急诊处理,生命体征平稳后,对照组给予常规西药治疗,研究组加用平脉定悸方。观察两组治疗前后的血清IL-1β、NF-κB及心率变异性指标变化,比较两组的治疗效果。结果 相比治疗前,两组治疗后LVEF水平升高,LVEDd、LVESd水平降低,并且组间对比研究组的LVEF更高,LVEDd及LVESd水平更低(P<0.05)。相比治疗前,两组治疗后的SDANN、SDNN、RMSSD水平均升高(P<0.05),组间对比研究组的SDANN、SDNN、RMSSD水平更高(P<0.05)。两组治疗后的血清IL-1β、NF-κB水平均较治疗前明显降低,且组间对比研究组的血清IL-1β、NF-κB水平更低(P<0.05)。研究组总有效率为92.31%,显著高于对照组的76.92%(P<0.05)。研究组随访期间的复发率显著低于对照组(P<0.05)。结论 在快速型心律失常的临床治疗中,平脉定悸方的加用可进一步减轻机体炎症反应,保护血管内皮功能,改善心率变异性,从而提高治疗效果。Objective:To explore the therapeutic effect of Pingmai Dingji Decoction on tachyarrhythmia and its influence on serum interleukin-1β(IL-1β),nuclear factor-κB(NF-κB)and heart rate variability.Methods:A total of 78 patients with acute onset of severe tachyarrhythmia admitted to our hospital were randomly divided into two groups(39 cases each).The control group was treated with conventional western medicine,and the study group was treated with Pingmai Dingji Decoction.The changes of serum IL-1β,NF-κB,heart rate variability in⁃dex were observed before and after treatment in the two groups,and the treatment effect of the two groups were compared.Results:Compared with before treatment,the LVEF level increased,LVEDd and LVESd levels de⁃creased after treatment in the two groups,and the comparison between groups showed that the LVEF level in the study group was higher,LVEDd and LVESd levels were lower(P<0.05).Compared with before treatment,the lev⁃els of SDANN,SDNN,and RMSSD were increased(P<0.05).The inter-group comparison showed that the levels of SDANN,SDNN,and RMSSD in the study group were higher(P<0.05).The level of serum IL-1βand NF-κB after treatment in both groups were significantly lower than that before treatment,and the comparison between groups showed that the levels of serum IL-1βand NF-κB in the study group were lower(P<0.05).The total ef⁃fective rate of the study group(92.31%)was significantly higher than that of the control group(76.92%)(P<0.05).The recurrence rate of the study group during the follow-up period was significantly lower than that of the control group(P<0.05).Conclusion:In the clinical treatment of tachyarrhythmia,the addition of Pingmai Dingji Decoction can further reduce the inflammatory reaction of the body,protect the vascular endothelial function,im⁃prove the heart rate variability,and improve the treatment effect.
