摘 要:目的探讨改进型移动数字X线摄影术(DR)联合成像对外周静脉置入中心静脉导管(PICC)术后及鼻空肠管置入(NTI)术后的实用价值。方法对530例PICC或NTI术后重症患者行移动DR摄影,分析置管情况,以单部位成像为参照与联合成像优质图像比率及辐射剂量值形成对比。结果530例中476例(89.81%)管头到位,47例(47/54,87.04%)需矫正后重摄定位;147例单独、联合成像中,联合成像辐射剂量[(5.98±1.32)mAs]明显低于单独成像[(7.43±0.81)mAs](P<0.05),而优质片率(129/147,87.76%)低于单独成像(136/147,92.52%)(P>0.05);2种成像方式仅在辐射剂量上有统计学差异。结论以改进式移动DR对PICC、NTI术后重症患者行联合成像,既能在原有基础上降低放射技师及环境辐射剂量,又能直观性显示管头位置,便于对置管误差及时调适,保证重症患者术后供药、供养,于临床具有重要实施价值。Objective To discuss the practical value of modified mobile digital radiography(DR)combined multiple-site imaging in patients who underwent peripherally inserted central catheter(PICC)and nasojejunal tube insertion(NTI).Methods A total of 530 sever patients underwent PICC or NTI were examined with mobile DR,and the catheter situations were analyzed.Taking single-site imaging as a reference,the ratio of high quality images and radiation dose value of single-site imaging were compared with those of combined multiple-site imaging.Results In the total 530 patients,476 cases(89.81%)had the tube head in place,in which 47 cases(47/54,87.04%)needed rephotography after correction.In 147 cases with single-site and combined multiple-site imaging,the radiation dose of combined multiple-site imaging was significantly lower than that of single-site imaging dose[(5.98±1.32)mAs vs(7.43±0.81)mAs](P<0.05).However,the rate of high quality image ratio of combined multiple-site imaging(129/147,87.76%)was lower than that single-site imaging(136/147,92.52%)(P>0.05).There was only a statistical difference in radiation dose between the two imaging methods.Conclusion Combined multiple-site imaging using modified mobile DR for PICC and NTI sever patients can not only reduce the radiation dose of radiographers and environmental on the original basis,but also visually display the position of tube head.It is convenient for adjustment of tube head error timely and guarantee the postoperative medicine supply and nutrition support for sever patients,which has important clinical implementation value.
