摘  要:目的基于仿真胸部体模探讨CT不同重建矩阵对肺部结节测量及胸部图像质量的影响。方法于PH-1仿真胸部体模中随机放置不同大小(3、5、8、10、12 mm)、不同密度(-630、-800、100 HU)共15个人工球形结节。采用联影uCT760对体模进行扫描,基于原始扫描数据采用3种不同矩阵(512×512、768×768、1024×1024)进行重建,获得A、B、C 3组图像。在后处理工作站上应用肺结节计算机辅助诊断(CAD)软件对结节进行分析,于轴位图像上测量结节最大截面长径、短径,并计算长短径误差和、失圆度和体积测量的绝对错误率(APE)。测量肺尖、胸廓最大层面、肺底均质区域的肺组织、气管隆嵴层面的胸椎旁软组织、主动脉弓和心脏最大层面的CT值及标准差(SD)值(即噪声值)。2位观察者采用双盲法对不同矩阵下图像质量及肺结节显示进行5分制评价。采用单因素方差分析比较各组肺结节长短径误差和、CT值及SD值的差异;2位医师图像主观评分的一致性采用Kappa检验,结节的失圆度、体积APE与图像的主观质量评分采用Friedman检验。结果3组间肺结节长短径误差和、失圆度及APE有显著性差异(P<0.05),并随着矩阵的增大而逐渐减小。3组间CT值差异无显著性差异(P>0.05)。SD值随矩阵增大而升高,差异有统计意义(P<0.001)。2位观察者主观评分一致性好(Kappa=0.71~0.90)。C组综合主观评分最高。结论高重建矩阵对肺部结节测量准确性高,肺部微细结构显示更佳,有助于肺部病变的精准诊断。Objective To investigate the effect of different CT reconstruction matrices on pulmonary nodules measurement and chest image quality based on anthropomorphic chest phantom.Methods Fifteen artificial spherical nodules with different sizes(3,5,8,10,and 12 mm)and different densities(-630,-800,and 100 HU)were randomly placed in PH-1 anthropomorphic chest phantom.The phantom was scanned by the uCT760 and reconstructed by using three different matrices(512×512,768×768,1024×1024)based on the raw scan data to obtain A,B,and C group of the images,respectively.The nodules were analyzed on a post-processing workstation using computer-aided diagnosis(CAD)software for pulmonary nodules.The long and short diameter of nodules were measured on the axial maximum cross-sectional images,and the sum of long and short diameter errors,out-of-roundness and absolute percentage error(APE)of volume measurements were calculated.CT and standard deviation(SD)values were measured at the apical lung,the largest level of the thorax,the homogeneous region of the base lung,the parasternal soft tissue at the level of the tracheal ridge,the aortic arch and the largest level of the heart,respectively.Two observers evaluated the image quality and pulmonary nodule display under different reconstruction matrices by using 5-point scale and double-blind method.One-way ANOVA was used to compare the differences in the errors sum in the long and short diameter of pulmonary nodules,CT values and SD values among the three groups.Kappa test was used to analyze the agreement of the subjective scores between the two observers.Friedman test was used to compare the subjective quality scores of the images,the out-of-roundness and volume APE of nodules.Results The error sum of the length and short diameter,out-of-roundness and APE were significantly different among the three groups(P<0.05),and gradually decreased with increasing of the reconstruction matrix.The CT values were not significantly different among the three groups(P>0.05).The SD values increased with
