摘  要:目的 探讨基于培训者培训(training the trainer to train, TTT)模式的学员教官制在战现场急救教学中的应用效果。方法 从某军医院校“红十字战救突击队”已接受教员培训的老队员中遴选学员教官(对照组),学员教官对新队员(试验组)进行战救培训及考核。研究者自行设计问卷对试验组进行教学效果评价,通过历史对照比较两组战救技术考核结果。结果 96.61%的学员愿意接受学员教官培养模式,77.97%的学员愿意成为学员教官,学员对授课质量的评价得分最高的条目是操作要点讲解(4.64±0.73),对每个授课模块的评价得分最高的项目是包扎(4.78±0.52),学员掌握情况自评得分最高的项目为包扎(4.27±0.84),学员最希望获取的教学能力为随机应变能力(4.81±0.47)。试验组止血与通气技能通过率分别为81.36%、 76.27%,表明大多数学员已基本掌握这两项技能。两组止血和通气考核通过率比较差异无统计学意义。质性评价中,学员表示学员教官上课生动有趣;学员们希望成为学员教官,沿用该模式;希望完善教学模式、增加培训时间。结论 TTT模式在战现场急救教学中具有良好的效果,受训学员对TTT模式接受度和参与度均较高,该模式在军医院校和基层部队均具有一定的推广价值。Objective To explore the application effects of the trainee-instructor system based on the training the trainer to train(TTT) model in the battlefield first aid teaching. Methods The cadet instructors(control group) were selected from the old members of the "Red Cross Combat Rescue Commandos" who had received the training of instructors in a military medical college, and the cadet instructors were invited to conduct combat rescue training for the new members(experimental group) to assess the combat rescue techniques of the trainees. The questionnaire designed by the researcher was used to evaluate the teaching effects of the experimental group on the cadets, and the results of the combat rescue technique assessment of the two groups were compared by historical comparison. Results 96.61% of the cadets were willing to accept the training mode of cadet instructors, and 77.97% of the cadets were willing to become cadet instructors, and the item with the highest score in the evaluation of teaching quality was the explanation of operating points(4.64±0.73), and the item with the highest score in the evaluation of each teaching module was the bandage(4.78±0.52), the item with the highest score of students′ self-evaluation of mastery was bandaging(4.27±0.84), and the teaching ability that students most hoped to acquire was adaptability(4.81±0.47). The passing rates of hemostasis and ventilation skills in the experimental group were 81.36% and 76.27% respectively, which indicated that most cadets had basically mastered these two skills, and there was no significant difference in passing rate of hemostasis and ventilation between the two groups. In the qualitative evaluation, cadets expressed that the instructors were lively and interesting in class;cadets hoped to become cadet instructors and follow this model;cadets hoped to improve the teaching model and increase the training time.
