椎间融合器联合自体髂骨在TLIF手术治疗腰椎退变性疾病中的优势分析

摘　　要：目的观察椎间融合器联合自体髂骨在TLIF手术治疗腰椎退变性疾病中的应用效果。方法选取2019年5月—2020年10月在该院接受TLIF手术治疗的腰椎退变性疾病患者为研究对象,并根据随机数字表法将其分为椎间融合器联合同种异体骨组(对照组)和椎间融合器联合自体髂骨组(观察组)。观察二组患者的手术时间、术中出血量、住院时间的差异,比较二组患者手术前后炎症因子水平、腰椎功能和并发症发生率的差异。结果二组患者的手术时间、术中出血量和术后住院时间比较差异无统计学意义(P>0.05);术后3个月,观察组患者椎间融合率为92.00%,高于对照组患者的64.00%,差异有统计学意义(χ^(2)值=11.422,P=0.001),二组患者术后6个月椎间融合率比较差异无统计学意义(χ^(2)值=1.383,P=0.240);二组患者手术前腰椎功能无差别,术后3个月,二组患者的腰椎功能得分均较术前增高(P<0.05),而二组间比较差异无统计学意义(P>0.05);二组患者术前炎症因子水平无差别,术后7天,二组患者的IL-6、hs-CRP和PCT水平较术前增高(P<0.05),二组间比较无差别(P>0.05);二组患者神经根损伤、切口感染和脑脊液漏发生率比较,差异无统计学意义(χ^(2)=0.344,P=0.558)。结论椎间融合器联合同种异体骨和椎间融合器联合自体髂骨在治疗腰椎退变性疾病中的效果相当,但椎间融合器联合自体髂骨的TLIF手术患者椎间融合率较高,具有良好的应用价值。Objective To observe the effect of intervertebral fusion cage combined with autologous iliac bone in TLIF surgery for the treatment of degenerative diseases of the lumbar spine.Methods The patients with degenerative lumbar spine disease who underwent TLIF surgery in our hospital from May 2019 to October 2020 were selected as the research objects,and they were divided into the intervertebral fusion cage combined with allograft bone group according to the random number table method(control Group)and interbody fusion cage combined with autologous iliac bone group(observation group).Observe the difference in operation time,intraoperative blood loss,and hospital stay between the two groups of patients,and compare the differences in the levels of inflammatory factors,lumbar spine function and the incidence of complications between the two groups before and after the operation.Results There was no significant difference in operation time,intraop-erative blood loss and postoperative hospital stay between the two groups of patients(P>0.05);3 months after operation,the intervertebral fusion rate of the observation group was 92.00%,which was higher than that of the control group 64.00%of patients,the difference was statistically significant(X^(2)=11.422,P=O.oo1),there was no significant difference in the intervertebral fusion rate between the two groups of patients at 6 months postoperatively(X^(2)=1.383,P=0.240);There was no difference in lumbar spine function be-tween the two groups of patients before operation.At 3 months after operation,the scores of lumbar spine function of the two groups were higher than those before operation(P<0.05),but there was no significant difference between the two groups(P>0.05);the two groups of patients before operation There was no difference in the levels of inflammatory factors.At 7 days after the operation,the levels of IL-6,hs-CRP and PCT in the two groups were higher than those before the operation(P<0.05),and there was no difference between the two groups(P>0.05);nerve root injury in
