妊娠合并肺动脉高压31例临床分析

摘　　要：目的探讨妊娠合并肺动脉高压患者的妊娠结局。方法收集2000年1月至2020年11月西安交通大学第一附属医院妇产科收治的妊娠合并肺动脉高压31例患者的临床资料,并进行回顾性分析。结果妊娠合并肺动脉高压患者原发病多为先天性心脏病,发生率为71.0%,其中重度肺动脉高压占54.5%;妊娠合并肺动脉高压患者剖宫产率87.1%,椎管内麻醉80.6%;早产率45.2%,流产率19.3%;胎儿病死率6.5%,新生儿窒息率3.2%,胎儿生长受限(FGR)发生率9.7%,均为重度肺动脉高压患者;重度肺动脉高压孕产妇合并心力衰竭、呼吸衰竭、ICU入住率高于轻度组(P均<0.05)。结论妊娠合并肺动脉高压原发疾病多为先天性心脏病,以肺动脉压力重度升高为主。妊娠合并重度肺动脉高压母儿并发症多,妊娠结局差。Objective To investigate the pregnancy outcome of pregnant women with pulmonary hypertension.Methods From January 2000 to November 2020,31 pregnant women who were treated with pulmonary hypertension in our hospital were retrospectively analyzed.Results Most of the primary cases of pregnancy with pulmonary hypertension were congenital heart disease,with an incidence of 71.0% among which severe pulmonary hypertension accounted for 54.5%.Cesarean section rate and intraspinal anesthesia were 87.1% and 80.6% respectively.The incidence of preterm delivery was 45.2% and the incidence of miscarriage was 19.3%.Fetal mortality was 6.5%,neonatal asphyxia was 3.2%,and fetal grouth restriction(FGR)was 9.7%,all of which were severe pulmonary hypertension.Severe pulmonary hypertension in pregnant women with heart failure,respiratory failure,and ICU occupancy rates were significantly higher than those in the mild group,with statistically significant differences(P all<0.05).Conclusion The primary disease of pregnancy with pulmonary hypertension was mostly congenital heart disease,mainly with severe pulmonary hypertension.Pregnancy with severe pulmonary hypertension was associated with more complications and poor outcomes.
