880份Ⅰ类切口病历围手术期预防性应用抗生素调查分析

摘　　要：目的 分析阳朔县人民医院Ⅰ类切口手术病历围手术期预防性应用抗生素的情况信息,为阳朔县人民医院临床围术期合理应用抗生素提供参考。方法 回顾性选取2020年于阳朔县人民医院行手术的880份Ⅰ类切口手术病历,对该880份病历围手术期预防性应用抗生素的信息进行合理性点评剖析,包括抗生素预防性使用的品种选择、用量用法、术前的给药时间、术中有无追加、预防用药疗程以及联合用药情况等。结果 880份Ⅰ类切口手术病历抗生素预防使用率为27.61%,预防性应用抗生素的243份病历中有56份病历抗生素使用不合理,占比23.05%,不合理率最高的是遴选药物不合理,其次为预防用药疗程不合理以及无指征预防用药。全部病历未发现有联合用药情况,但单份病历含2个或以上不合理用药情况的有12份,占比21.43%(12/56)。结论 阳朔县人民医院Ⅰ类切口手术患者围手术期预防性应用抗生素比率符合国家卫健委要求,但仍存在诸多不合理问题,有待进一步提升合理用药水平。Objective To analyze the information of prophylactic use of antibiotics during perioperative period in the medical records of class 1 incision surgery in the people's hospital of Yangshuo county,so as to provide reference for promoting the rational use of antibiotics during perioperative period in clinical practice in the people's hospital of Yangshuo county.Methods The medical records of 880 patients undergoing class 1 incision surgery in the people's hospital of Yangshuo county in 2020 were retrospectively analyzed,and the rationality of perioperative preventive application of antibacterial drugs was reviewed and analyzed,including the selection of preventive use varieties,usage and dosage,preoperative dosing time,hether it was added during operation,preventive drug treatment course and combination drug use,etc.Results 880 medical records of Class I incision surgery the rate of antibiotic prophylaxis was 27.61%,Among the 243 medical records of prophylactic antibiotics,56 cases were irrational,accounting for 23.05%.The highest irrational rate was irrational drug selection,followed by irrational course of prophylactic drugs and no indication of prophylactic drugs.No combination drug use was found in all medical records,but 12 cases(21.43%)had two or more irrational drug use cases in a single medical record.Conclusion The ratio of perioperative prophylactic use of antibacterial drugs in patients undergoing class 1 incision surgery in the people's hospital of Yangshuo county meets the requirements of the national health and family planning commission,but there are still many unreasonable problems,which need to be further improved.
