张炜运用麻黄附子地黄饮治疗婴幼儿喘息性支气管炎经验

摘　　要：张炜教授认为,婴幼儿喘息性支气管炎以肾气虚弱为本,寒邪阻滞,痰浊瘀滞为标。“实则泻之,虚则补之”,治以温养肾气、发散伏寒、化痰活血,用《伤寒论》麻黄附子细辛汤、北宋赵佶《圣济总录》地黄饮、明朝韩天爵《韩氏医通》三子养亲汤、清代林佩琴《类证治裁》桃红饮加减,组成麻黄附子地黄饮合方治疗,方中麻黄附子细辛汤搜剔伏寒、地黄饮滋阴补肾、三子养亲汤温化伏痰、桃红饮活血化瘀。全方有温补肾气,宣肺散寒,化痰活血,止哮平喘之效,契合该病病机。Professor Zhang Wei believes that infant asthmatic bronchitis is due to weakness of kidney qi as the radical,blockage of cold evil and chronic phlegm stasis as the incidental."In a case of excess,purge or reduce it;In a case of deficiency,reinforce or tonify it."The treatment focuses on warming and nourishing the kidney qi,diverting the latent cold,resolving phlegm and promoting blood.That's why Ephedra and Aconite and Asarum Decoction from Treatise on Cold Damage Diseases,Rehmannia Drink from Comprehensive Record-ing of Divine Assistance by Zhao Ji in the Northern Song Dynasty,Three-Seed Filial Devotion Decoction from Han's Clear View of Medi-cine by Han Tianjue in the Ming Dynasty,Modified Peach Kernel and Carthamus Drink from Categorized Patterns with Clear-cut treat-ments by Lin Peiqin in the Qing Dynasty,are used to form Ephedra and Aconite and Asarum Decoction.In this compound formula,Ephedra and Aconite and Asarum Decoction can find and remove latent cold;Rehmannia Drink can nourish yin and the kidney;Three-Seed Filial Devotion Decoction can warm and resolve latent phlegm;Peach Kernel and Carthamus Drink can promote blood circulation and remove blood stasis.The whole formula has the effect of warming and tonifying kidney qi,dispersing the lung and dispelling cold,re-solving phlegm and promoting blood circulation,stopping and calming asthma,which fits the pathogenesis of the disease.
