补肾促排卵汤联合针灸对未破卵泡黄素化综合征患者激素水平和卵巢血流动力学的影响

摘　　要：目的:观察补肾促排卵汤联合针灸治疗未破卵泡黄素化综合征(luteinized unruptured follicle syndrome, LUFS)的临床疗效及其对患者激素水平和卵巢血流动力学的影响。方法:将100例LUFS患者按照随机数字表法分为观察组和对照组,每组各50例。对照组在卵泡直径>16 mm时给予绒毛膜促性腺激素针,肌肉注射;观察组在对照组治疗的基础上给予补肾促排卵汤联合针灸治疗。比较两组患者的临床疗效、排卵率及妊娠率;检测两组患者治疗前后激素水平及卵巢血流动力学指标[卵巢动脉的搏动指数(pulsatility index, PI)和阻力指数(resistance index, RI)]。结果:对照组有效率为72.00%,观察组有效率为86.00%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后雌二醇水平低于本组治疗前,黄体生成素高于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后PI、RI低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。观察组妊娠率及排卵率高于对照组,差异具有统计学意义(P<0.05)。结论:补肾促排卵汤联合针灸治疗LUFS,可改善患者的激素水平及卵巢血流动力学指标,提高排卵率及妊娠率。Objective:To observe the clinical efficacy of Kidney-Tonifying and Ovulation-Promoting Decoction combined with acupuncture and moxibustion on patients with luteinized unruptured follicle syndrome(LUFS)and the influence on their hormone levels and ovarian hemodynamics.Methods:A total of 100 LUFS patients were divided into the observation group and the control group according to the random number table method,with 50 cases in each group.The control group was given chorionic gonadotropin for intramuscular injec-tion when the follicle diameter was more than 16 mm.The observation group was given Kidney-Tonifying and Ovulation-Promoting De-coction combined with acupuncture and moxibustion on the basis of the control group.The clinical efficacy,ovulation rate and pregnancy rate of the two groups were compared.Hormone levels and ovarian hemodynamic indexes,[including pulsatility index(PI)and resist-ance index(RI)] before and afer treatment were detected in the two groups.Results:The effective rate of the control group was 72.00%,and that of the observation group was 86.00%.The difference between the two groups was statistically significant(P<0.05).The estradiol level of the two groups after treatment was lower than that before treatment,while the luteinizing hormone level was higher than that before treatment,and all the differences between the two groups were statistically significant(P<0.05).PI and RI af-ter treatment were lower than those before,and the observation group's PI and RI were lower than those of the control group,and all the diferences were statistically significant(P<0.05).The pregnancy rate and ovulation rate of the observation group were higher than those of the control group,and all the differences were statistically significant(P<0.05).Conclusion:Kidney-Tonifying and Ovulation-Promoting Decoction combined with acupuncture and moxibustion for the treatment of LUFS can improve the hormone level and ovarian hemodynamic index,and increase the ovulation rate and pregnancy rate.
