黄芪桂枝五物汤联合聪脑通络法针刺对小儿痉挛型脑瘫痉挛指数及脑血流动力学指标的影响

摘　　要：目的:观察黄芪桂枝五物汤联合聪脑通络法针刺对小儿痉挛型脑瘫痉挛指数及脑血流动力学指标的影响。方法:选取2018年1月至2022年8月南阳市骨科医院诊治的小儿痉挛型脑瘫186例,按照数字随机表法分成常规组和试验组,每组各93例。常规组给予黄芪桂枝五物汤治疗,试验组给予黄芪桂枝五物汤联合聪脑通络法针刺治疗。观察两组患儿治疗前后粗大运动功能、格塞尔婴幼儿发展量表、脑血流动力学指标、痉挛指数及临床疗效。结果:试验组有效率为94.62%,常规组有效率为82.79%,试验组有效率高于常规组,差异有统计学意义(P<0.05);试验组治疗后粗大运动功能评分高于常规组,差异有统计学意义(P<0.05);试验组治疗后格塞尔婴幼儿发展量表评分高于常规组,差异有统计学意义(P<0.05);试验组治疗后脑血流动力学指标优于常规组,差异有统计学意义(P<0.05);试验组治疗后痉挛指数低于常规组,差异有统计学意义(P<0.05)。结论:黄芪桂枝五物汤联合聪脑通络法针刺治疗小儿痉挛型脑瘫能改善患儿脑血流动力学指标、粗大运动功能,降低脑瘫痉挛指数,促进患儿早日恢复至正常水平。Objective:To observe the influence of Astragalus and Cinnamon Twig Five Substances Decoction combined with acupuncture for improving brain and dredging collaterals on spasticity indexes and cerebral hemodynamic index of children with spastic cerebral pal-sy.Methods:A total of 186 children with spastic cerebral palsy diagnosed and treated in Nanyang Orthopedic Hospital from January 2018 to August 2022 were selected and divided into the conventional group and the experimental group,with 93 cases in each group ac-cording to the random number table method.The conventional group was treated with Astragalus and Cinnamon Twig Five Substances Decoction,while the experimental group was treated with Astragalus and Cinnamon Twig Five Substances Decoction combined with acu-puncture for improving brain and dredging collaterals.Then the changes in gross motor function,Gesell infant development scale,cere-bral hemodynamic index,spasticity index before and after treatment and clinical efficacy were observed.Results:The effective rate of the experimental group was 94.62%and that of the conventional group was 82.79%.The effective rate of experimental group was higher than that of conventional group,and the difference was statistically significant(P<0.05).After treatment,the gross motor function score of the experimental group was higher than that of the conventional group,and the difference was statistically significant(P<O.05);The score of Gesell infant development scale of the experimental group was higher than that of the conventional group,and the difference was statistically significant(P<0.05);The cerebral hemodynamic indexes of the experimental group were better than those of the conventional group,and all the differences were statistically significant(P<0.05);The spasticity index of the experimental group was lower than that of the conventional group,and the difference was statistically significant(P<0.05).Conclusion:In terms of spastic cerebral palsy in children,Astragalus and Cinnamon Twig Five Substances Decoction combi
