基于Notch信号通路探讨外用中药在慢性难愈性创面中的应用

摘　　要：Notch通路作为人体内重要的信号转导途径,参与多种生理活动,可通过调节巨噬细胞极化、促进炎症反应、促进血管新生、加速创面再上皮化等多种途径参与慢性难愈性创面的修复。目前,关于Notch信号通路在慢性难愈性创面中的研究仍存在以下问题:(1)有关Notch信号通路在慢性难愈性创面中的研究不统一,通过激活或是抑制该信号通路从而达到治疗目的仍有争议,这可能与研究所使用的动物模型、创面类型与分期、具体作用机制有关;(2)通过该信号通路治疗慢性难愈性创面作用的靶点尚不清晰,仅以少数几个经典靶点蛋白做支撑,无法系统判断该通路在慢性创面愈合中的参与程度,与其他信号通路之间的交互作用同样缺乏系统研究;(3)大部分中药复方制剂调控Notch信号通路发挥作用的具体机制尚不明朗,多是以某一下游或上游关键靶点蛋白表达水平做支撑佐证相应的观点,少有对多个靶点蛋白进行系统的研究。今后,还需进一步明确各靶点间的相互作用,对中药靶向Notch通路治疗慢性难愈性创面进行更为系统的研究,有助于该病特异性治疗药物的研发。Notch pathway as an important signal transduction pathway in human body,is involved in a variety of physiological activities.It can participate in the repair of chronic and refractory wounds by regulating the polarization of macrophages,promoting inflammation and angiogenesis,accelerating wound re-epithelialization and other ways.However,there are currently the following problems:(1)Stud-ies on Notch signaling pathway in chronic refractory wounds are inconsistent,and it is still controversial whether Notch signaling pathway can be activated or inhibited to achieve therapeutic purposes,which may be related to the animal models used in the research,wound type and stage,and specific mechanism of action;(2)The targets of the treatment of chronic refractory wounds by certain signaling pathway is not clear,and only a few classical target proteins are used as support,so the participation of a pathway in chronic wound healing cannot be systematically judged,and the interaction between this pathway and other signaling pathways is also lacking in system-atic studies;(3)The specific mechanism by which Notch signaling pathway is regulated by most Chinese herbal compound preparations is still unclear,and the corresponding views are mostly supported by the expression level of a certain downstream or upstream key target protein,and few systematic studies have been conducted on multiple target proteins.In the future,it is necessary to further clarify the in-teraction between various targets,and conduct a more systematic study on the treatment of chronic refractory wounds by targeting Notch pathway with Chinese medicines,which is conducive to the development of specific therapeutic medicines for this disease.
