升降散治疗呼吸系统疾病作用机制研究进展

摘　　要：升降散的主要组成药物有僵蚕、蝉蜕、大黄、片姜黄四味,四药相伍,寒温并用、升降互济、透泄并举,共奏升清降浊、散风清热之功。升降散可用于治疗结核性胸膜炎、急性呼吸窘迫综合征、肺炎、急性扁桃体炎、流行性感冒、支气管哮喘等。但升降散在呼吸系统疾病中具体作用方式尚不明晰,研究较为零散,当前的研究多采用动物实验与临床研究相结合的方式,多中心、大样本的临床随机试验和动物实验较少,对升降散中有效成分的研究进展较慢,药理作用研究不完善,涉及蛋白质组学、基因组学、转录组学、代谢组学、血清药理学、网络药理学等研究较少。今后,应注意对升降散的用药组方规律、复方配伍机制、复方量效等予以明确;关注升降散有效成分通过灌肠、涂擦等外治法发挥治疗作用的情况;继续扩大临床应用范围,制定统一的疗效判定标准,进行多中心大样本的临床对照试验和动物实验。The main components of Ascending-Descending Powder are the following four herbs:Jiangcan(Bombyx Batryticatus),Chan-tui(Periostracum Cicadae),Dahuang(Radix et Rhizoma Rhei)and Pianjianghuang(Rhizoma Wenyujin Concisum),which are com-bined,used for ascending lucid yang and descending turbidity,dispersing wind and removing heat by using cold and warm natures,as-cending and descending helping each other,and penetrating and purging together.Ascending-Descending Powder can be used to treat tu-berculous pleurisy,acute respiratory distress syndrome,pneumonia,acute tonsllitis,influenza,bronchial asthma,etc.However,the spe-cific mode of action of Ascending-Descending Powder in respiratory diseases is still unclear,and the research is relatively scattered.The current research mostly adopts the combination of animal experiments and clinical research.There are fewer multi-center,large-sample clinical randomized trials and animal experiments.The research progress of the effective components of Ascending-Descending Powder is slow,and the research on pharmacological effects is incomplete,involving proteomics,genomics,transcriptome,metabolomics,serum pharmacology,network pharmacology,etc.In the future,research should pay attention to clarifying the formula composition rules,com-pound compatibility mechanisms,and compound dosage and efficacy of Ascending-Descending Powder,and focus on the therapeutic effects of the effective components of Ascending-Descending Powder through external treatment methods such as enema and smearing,and continue to expand the scope of clinical application,develop unified efficacy evaluation standards,and conduct multi-center large-sample clinical controlled trials and animal experiments.
