居住安排对医养结合机构老年人自评健康状况的影响研究

摘　　要：目的探讨居住安排对医养结合机构入住老年人自评健康的影响,为我国老年人的居住安排研究提供科学参考。方法采用分层随机抽样方法,利用自制问卷于2017年6月-2018年8月在山东省青岛市7个区抽取43家医养结合机构入住的1907名老年人进行问卷调查,应用χ^(2)检验对计数资料进行单因素分析,利用有序logistic回归对老年人自评健康的影响因素进行分析。结果被调查的医养结合机构入住老年人的自评健康属于中等水平。有序logistic回归分析结果显示,与子女就近居住、生活感到满意、无焦虑抑郁感、给予老年人最主要的代际支持为经济支持以及居住的养老机构内设医疗机构的老年人自评健康较好;经济状况越差、婚姻状况为非在婚的老年人自评健康较差(均P<0.05)。结论居住安排与老年人自评健康显著相关。应当健全社会保障体系,倡导子女等家庭成员与入住机构的老年人近邻居住,增强对机构老年人的社会关爱,创新和发展不同类型医养结合机构服务模式,提高老年人收入水平,维护和改善老年人的健康状况。Objective To explore the impact of living arrangement on the self-rated health of the elderly in integrated medical and nursing institutions,so as to provide scientific reference for the study of living arrangement of the elderly in China.Methods A total of 1907 elderly people from 43 integrated medical and nursing institutions were selected from 7 districts of Qingdao City Shandong Province from June 2017 to August 2018 by stratified random sampling method with a self-designed questionnaire.χ^(2) test were used to analyze the enumeration data by single factor analysis.Ordinal logistic regression was used to analyze the influencing factors of self-rated health of the elderly.Results The self-rated health of the elderly in the medical and nursing institutions was of medium level.The results of ordinal logistic regression analysis showed that the elderly living nearby with their children,satisfied with life,no anxiety and depression,the most important intergenerational support for the elderly is economic support,and the elderly living in medical institution within the pension institutions have better self-rated health.The health status of the elderly with poor economic status and non-marital status was worse(all P<0.05).Conclusion There is a significant correlation between living arrangement and self-rated health of the elderly.We should improve the social security system,advocate that children and other family members live close to the elderly in institutions,strengthen social care for the elderly in institutions,innovate and develop different types of service modes of integrated medical and nursing institutions,improve the income level of the elderly,and maintain and improve the health status of the elderly.
