断指再植术后应用当归四逆汤的临床疗效观察

摘　　要：目的探讨断指再植术后应用当归四逆汤的临床疗效。方法240例手指离断行断指再植术后的患者随机分为对照组与治疗组各120例。对照组常规应用抗炎、抗凝、抗痉挛等经典西药3抗治疗;治疗组在对照组常规治疗基础上加用当归四逆汤颗粒剂,1剂·d^(-1),分2次水冲服,1疗程10 d。1疗程后观察患者再植指成活率、感染率、血运情况、疼痛评分。结果治疗组与对照组再植指成活率、感染率差异无统计学意义(P>0.05);治疗组血运情况、疼痛评分优于对照组(P<0.05)。结论当归四逆汤有抗凝、抗血栓形成、扩张末梢血管、增加血管灌流量及镇痛、抗炎、解痉作用,可明显改善再植指的血液循环,减轻患者疼痛,符合断指再植术后的3抗治疗原则,临床疗效良好。Objective To explore the clinical effect of Danggui Sini decoction after replantation of severed fingers.Methods A total of 240 patients with severed fingers undergoing replantation were randomly divided into a control group and a treatment group,with 120 cases in each group.Patients in the control group were routinely treated with classic western medicine treatment of anti-inflammatory,anticoagulant,anti-spasmodic;Patients in the treatment group were treated with Danggui Sini decoction granules on the basis of routine treatment in the control group.Half a dose for two times in a day was administrated with water,and 1 course lasted 10 days.After 1 course of treatment,the survival rate,infection rate,blood supply and pain score of the replanted fingers were observed.Results There was no significant difference in the survival rate and infection rate of replanted fingers between the treatment group and the control group(P>0.05).The blood supply and pain score of the treatment group were better than those of the control group(P<0.05).Conclusion Danggui Sini decoction has the effects of anticoagulation,antithrombosis,dilatation of peripheral blood vessels,increase of vascular perfusion flow,analgesia,anti-inflammation and spasmolysis.It can significantly improve the blood circulation of replanted fingers and reduce the pain of patients,which is in line with treatment principles after replantation of severed fingers and has good clinical efficacy.
