摘  要:目的 探讨喉部超声在甲状腺良性结节热消融术中实时评估声带功能的可行性和准确度。方法 选取接受消融治疗的良性甲状腺结节患者42例,应用喉部超声监测声带功能。通过喉镜验证声带麻痹(VCP),分析喉部超声实时评估甲状腺结节热消融术中声带功能的可行性和准确度。结果 喉部超声监测声带结构的可行性为100%;喉部超声诊断VCP的准确度为95%,灵敏度为80%,特异度为97.3%,误诊率为2.7%,漏诊率为2.5%;Kappa检验证实喉部超声诊断VCP与喉镜一致性高(P<0.001)。结论 喉部超声是一种高度可行、准确的评估甲状腺结节热消融时声带功能的方法,其在术中可实时、快速地向操作者反馈声带状况。Objective To explore the feasibility and accuracy of laryngeal ultrasound in real-time evaluation of vocal fold function during thermal ablation of benign thyroid nodules.Methods A total of forty-two patients with benign thyroid nodules who underwent ablation treatment at the included,and vocal cord function was monitored by laryngeal ultrasound.Vocal cord paralysis(VCP)was verified by laryngoscopy,and the feasibility and accuracy of laryngeal ultrasound for real-time assessment of vocal cord function during thermal ablation of thyroid nodules were analyzed.Results The feasibility of laryngeal ultrasound monitoring vocal core structures was 100%.The accuracy,sensitivity,specificity,misdiagnosis and underdiagnosis rate of laryngeal ultrasound diagnosis of VCP was 95%,80%,97.3%,2.7%and 2.5%,respectively.Kappa test confirmed the high consistency between VCP diagnosed by laryngeal ultrasound and laryngoscopy(P<o.oo1).Conclusions Laryngeal ultrasound is a highly feasible and accurate method to evaluate vocal cord function during thermal ablation of thyroid nodules,which can provides real-time and rapid intraoperative feedback to the operator on vocal fold status.
