摘  要:目的 探讨超声评估阻塞性睡眠呼吸暂停低通气综合征(OSAHS)颈部气管可行性及应用价值。方法 对174例OSAHS患者颈部气管进行超声检查,测量不同呼吸状态颈部气管横径与Muller状态颈部气管上下移动距离。将超声参数与身高等指标进行相关性分析,对比分析不同严重程度OSAHS组间超声参数差异,利用回归分析OSAHS病情发展的危险因素。结果 超声可显示颈部气管环前壁结构,颈部气管平静呼吸状态下显示率为100%,Muller状态下横切及纵切扫查显示率分别为88.50%和97.70%。轻、中、重度OSAHS组间超声参数存在差异(P<0.05)。年龄、体质量指数(BMI)、Muller状态颈部气管上下移动距离是OSAHS病情发展的危险因素(95%CI分别为0.94~1.0、1.06~1.31和1.01~1.15,P<0.05)。结论 超声可评估OSAHS颈部气道,可为其发病机制等研究及诊疗提供重要依据。Objective To explore the feasibility and application value of sonography in the evaluation of cervical trachea in patient with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods A total of 174 patients with OSAHS were examined by ultrasonography,and the parameters including the peak tracheal displacement with Muller maneuver,the lateral diameter of cervical trachea during different breathing status were obtained for statistic analysis and compared among groups.The correlation analysis of ultrasonic parameters with height and other items was conducted.Logistic regression model was used to identify risk factors for serious of OSAHS.Results The anterior wall of cervical trachea was well shown on sonography.The detection rates of cervical trachea were 1oo%(quiet breath),88.50%(Muller,transection section),97.70%(Muller,longitudinal section),respectively.There were significant statistically differences in ultrasound parameters among the mild,moderate,and severe OSAHS groups(P<0.05).The peak tracheal displacement with Muller maneuver,BMI and age were risk factors for the severity development of OSAHS(95%CI:0.94-1.0,1.06-1.31 and 1.01-1.15;P<0.05).Conclusions Ultrasonography can be used to assess the cervical trachea in patients with OSAHS,which can provide important information for the research on pathogenesis,diagnosis,and treatment of OSAHS.
