摘 要:目的 探讨组织运动二尖瓣环位移自动追踪(TMAD)技术评价不同程度冠状动脉病变患者左心室收缩功能。方法 选取冠心病(CHD)患者90例,健康体检者40例作为对照组,根据Gensini评分将CHD患者分为3组:轻度组(1~25分)28例、中度组(26~50分)34例、重度组(>50分)28例,测量TMAD参数与实时三维超声心动图(RT-3DE)参数、所需要的时间,比较各组间TMAD参数与RT-3DE参数的差异,并探讨2种参数之间的相关性。结果 与对照组相比,轻度、中度、重度组TMAD参数逐渐减低,差异均有统计学意义(P<0.05或P<0.01),轻度组实时三维超声心动图(RT-3DE)参数与对照组相比差异没有统计学意义(P>0.05),中度、重度组RT-3DE参数差异均有统计学意义(P<0.01);与轻度组相比,中度、重度组TMAD参数及RT-3DE参数差异均有统计学意义(P<0.01);与中度组相比,重度组TMAD参数及RT-3DE参数差异均有统计学意义(P<0.01);与RT-3DE相比,TMAD分析时间明显缩短(P<0.05);TMAD参数与左室射血分数(LVEF)相关性良好(P<0.01)。结论 TMAD可识别不同程度冠状动脉病变患者左心室收缩功能障碍,为临床早期发现CHD患者心功能异常提供了一种分析简捷、快速的方法。Objective This study aims to evaluate of left ventricular systolic function in patients with different levels of coronary heart disease using tissue motion tracking of mitral annular displacement(TMAD).Methods A total of 90 patients with coronary heart disease(CHD)and 40 healthy people as the control group were selected and divided CHD patients into three groups based on the Gensini score:mild(1-25 points)with 28 patients,moderate(26-50 points)with 34 patients,and severe(>50 points)with 28 patients.TMAD parameters and real time 3-dimensional echocardiography(RT-3DE)parameters were measured,and the time required for analysis was compared.Additionally,the differences and correlation between TMAD parameters and RT-3DE parameters among the groups were explored.Results Compared to the control group,TMAD parameters gradually decreased in the mild,moderate,and severe groups,with statistical significance(P<0.05 or P<0.01).The RT-3DE parameters in the mild group were not significantly different with the control group(P>0.05),while the RT-3DE parameters in the moderate and severe groups showed statistically difference with the control group(P<0.o1).There were statistically significant differences in TMAD parameters and RT-3DE parameters between the moderate and severe groups compared to the mild group(P<o.O1).Furthermore,the severe group had statistically significant differences in TMAD parameters and RT-3DE parameters compared to the moderate group(P<0.O1).The TMAD analysis time was significantly shorter compared to RT-3DE(P<0.05).There was a good correlation between TMAD parameters and left ventricular ejection fraction(LVEF)(P<O.O1).Conclusions TMAD parameters can identify left ventricular systolic dysfunction in patients with varying degrees of coronary artery disease.This provides a simple and fast analysis method for the early detection of cardiac dysfunction in patients with coronary heart disease.
