摘 要:目的应用分层应变联合压力应变环(PSL)技术定量评价先兆子痫(PE)患者左心室功能变化,探讨各参数对PE的诊断效能。方法选取先兆子痫(PE)患者78例,分为非重度组(NSPE组,38例)和重度组(SPE组,40例),另选取45例健康孕妇作为对照组。比较3组间各参数间差异及诊断效能。结果(1)与对照组相比,NSPE组整体做功指数(GWI)、整体有用功(GCW)、整体无用功(GWW)增加,整体做功效率(GWE)、心内膜下心肌整体纵向应变(GLSendo)降低(P<0.05);与对照组和NSPE组相比,SPE组GWI、GCW、GWW增加,GWE、GLSendo、中层心肌整体纵向应变(GLSmid)和心外膜下心肌整体纵向应变(GLSepi)降低(P<0.05)。(2)受试者工作特征(ROC)曲线显示GLSendo、GWE联合的曲线下面积(AUC)最大,为0.934。结论分层应变与PSL技术可以早期敏感地检测PE患者左心室心肌功能变化;GLSendo和GWE两者联合诊断效果更优。Objective To quantitatively evaluate the changes of left ventricular strain and myocardial work in patients with preeclampsia(PE)by applying the Layer-specific strain technique combined with pressure-strain loop(PSL),and to explore the diagnostic efficacy of each parameter in predicting PE.Methods A total of seventy-eight patients with PE were divided into non-severe(NSPE)and severe(SPE)groups according to their different severity,and another 45 healthy pregnant women with matching age and gestational weeks were selected as control group.The differences of parameters and diagnostic efficacy among the three groups were compared.Results①Compared with the control group,the global work index(GWI),global constructive work(GCW)and global wasted work(GWW)in NSPE group were increased,while the overall work efficiency(GWE)and overall longitudinal strain of subendocardial myocardium(GLSendo)were decreased(P<0.05).Compared with the control group and NSPE group,GWI,GCW and GWW in SPE group were increased,while GWE,GLSendo,global longitudinal strain of medial myocardium(GLSmid)and global longitudinal strain of subepicardial myocardium(GLSepi)were decreased(P<0.05).②Receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of the combination of GLSendo and GWE was the largest,0.934.Conclusions Layer-specific strain technique and PSL technique can detect the changes of left ventricular myocardial function sensitively in PE patients at an early stage.The combination of GLSendo and GWE has a better diagnostic effect.Download(CAJ format)Download(PDF format)
