新生儿复杂型卵巢囊肿的预后与超声特点分析

摘　　要：目的探讨新生儿复杂型卵巢囊肿的预后及超声特点。方法回顾性分析新生儿外科收住的15例复杂型卵巢囊肿患儿资料,对其产前、产后超声特征、术中情况、治疗、病理结果进行分析。结果①15例女患儿,新生儿期发现的卵巢囊肿16个,按术中探查时不同类型分3类:囊肿未扭转(A组)2个,14个卵巢囊肿并蒂扭转,其中4个囊肿发生自截(B组),10个囊肿未自截(C组)。②未扭转的囊肿中提示产后超声见多分隔样的特征100%(2/2),自截的囊肿中提示产前超声多呈无回声,囊内固体,单分隔75%(3/4),产后超声多呈液体-碎片分层征75%(3/4),25%(1/4)为多分隔。扭转未自截的囊肿中提示产前超声示30%(3/10)为单纯型卵巢囊肿,10%(1/10)为囊内固体,10%(1/10)为液体-碎片分层征,20%(2/10)为单分隔,40%(4/10)为多分隔,产后超声示50%(5/10)为液体-碎片分层征,40%(4/10)为多分隔。③病变平均时间跨度:未扭转的囊肿为7(4~10)d,自截的囊肿为106(49~153)d,扭转未自截的囊肿为34(10~80)d。结论新生儿复杂型卵巢囊肿高度提示卵巢扭转;多分隔,或液体-碎片分层征是卵巢囊肿并扭转中-晚期比较特异度的超声特征。如扭转时间超过7周的卵巢囊肿,需考虑自截。Objective To explore the prognosis and ultrasound(US)characteristics of complex ovarian cysts in neonates.Methods The data of 15 infants with complex ovarian cysts in were retrospectively analyzed.The prenatal and postnatal ultrasound features,intraoperative conditions,treatment and pathological findings of the patients were analyzed.Results ①There were 16 ovarian cysts found in 15 female neonates,which were divided into 3 groups according to different types:2 cysts without torsion(group A),14 ovarian cysts with torsion,of which 4 cysts were autoamputation(group B),and 10 cysts were confirmed torsion without autoamputation(group C).②In cysts without torsion,10o%(2/2)of postnatal US showed multiple compartmentalized features;in autoamputation cysts,the postnatal US mostly showed no echo,solid cyst,single compartmentalized 75%(3/4),and 25%(1/4)multi-compart mentalized.Torsion of untruncated cysts suggested that prenatal US showed simple cysts in 30%(3/10),solid cyst in 10%(1/10),fluid-fragment level in 10%(1/10),simple separation in 20%(2/10),and multi separation in 40%(4/10).Postnatal US was 50%(5/10)fluid-fragment level and 40%(4/10)multi separation.③The mean time was 7(4-10)days for cysts without torsion,106(49-153)days for cysts with autoamputation,and 34(10-80)days for cysts were confirmed torsion without autoamputation.Conclusions Complex ovarian cysts in neonates are highly suggestive of ovarian torsion;Multiple compartmentalization or fluid-debris levels are ultrasound features specific to ovarian cysts with torsion in the middle and late stages.Autoamputation should be considered for ovarian cysts with torsion longer than 7 weeks.
