摘　　要：目的:探讨原发性高血压不同血压昼夜节律患者脉图参数特征。方法:选择符合纳入标准的原发性高血压患者133例,其中杓型血压患者20例(杓型组),非杓型血压患者65例(非杓型组),反杓型高血压患者48例(反杓型组);另选择同期血压正常的健康体检者61例作为健康对照组。应用SMART-Ⅰ型中医数字化脉象分析仪分析原发性高血压不同血压昼夜节律组及健康对照组的脉图参数差异。结果:杓型组患者脉图参数t_(1)、h_(3)/h_(1)较健康对照组显著升高(P<0.05);非杓型组患者脉图参数h_(3)、t_(1)、t_(4)、t_(5)、h_(3)/h_(1)较健康对照组显著升高(P<0.05),h_(4)/h_(1)较健康对照组显著降低(P<0.05);反杓型组患者脉图参数t_(1)、t_(4)、t_(5)、h_(3)/h_(1)、w/t较健康对照组显著升高(P<0.05);反杓型组患者脉图参数h_(4)/h_(1)显著高于非杓型组(P<0.05)。结论:原发性高血压不同血压昼夜节律患者脉图参数有差异,提示脉图参数可作为评估原发性高血压不同血压昼夜节律患者血管功能状态的指标之一。Objective:To investigate the pulse chart parameters of patients with different blood pressure circadian rhythm of primary hypertension.Methods:We selected 133 patients with primary hypertension who met the inclusion criteria,20 dipper hypertension,65 non-dipper hypertension,and 48 inverse dipper hypertension.Sixty-one patients with normal blood pressure at the same period were selected as healthy control groups.SMART-Ⅰdigital pulse analyzer was used to analyze the difference of pulse chart parameters between the primary hypertension group with different blood pressure circadian rhythm and the healthy control group.Results:Pulse chart parameters t_(1)and h_(3)/h_(1)in dipper hypertension group were significantly higher than those in healthy control group(P<0.05).Pulse chart parameters h_(3),t_(1),t_(4),t_(5),and h_(3)/h_(1)of patients in non-dipper hypertension group were significantly higher than those in healthy control group(P<0.05),however,h_(4)/h_(1)was significantly lower(P<0.05).Pulse chart parameters t_(1),t_(4),t_(5),h_(3)/h_(1),w/t in patients with inverse dipper hypertension were significantly higher than those in healthy control group(P<0.05).Pulse chart parameter h_(4)/h_(1)in patients with inverse dipper hypertension was significantly higher in patients with non-dipper hypertension.Conclusion:The pulse chart parameters in patients with essential hypertension with different blood pressure circadian rhythm are different.The results suggest that pulse chart parameters can be used as one of the indexes of evaluating vascular functional status in different blood pressure circadian rhythm of primary hypertension.
