龙槐止血汤联合咖啡酸片治疗湿热下注型混合痔术后并发症临床研究

摘　　要：目的:观察龙槐止血汤联合咖啡酸片治疗湿热下注型混合痔术后并发症的疗效。方法:选择湿热下注型混合痔术后患者60例,随机分为观察组和对照组各30例。对照组患者给予咖啡酸片及中药安慰剂,观察组患者给予咖啡酸片和龙槐止血汤。术后1 d、3 d、7 d、14 d评估两组患者出血评分、黏膜评分、疼痛评分和水肿评分;术后第30天对两组患者进行随访,收集患者创面愈合时间;比较两组患者术后至痊愈期间尿潴留情况。结果:两组患者术后出血评分、黏膜评分均呈下降趋势,术后7 d、14 d,观察组出血评分和黏膜评分均低于对照组(P<0.05)。两组患者疼痛评分均于术后3 d出现峰值,之后持续下降;术后3 d、7 d、14 d,观察组患者疼痛评分均低于对照组(P<0.05)。两组患者术后水肿评分均呈下降趋势,术后7 d、14 d,观察组水肿评分低于对照组(P<0.05)。随访发现,观察组平均愈合时间短于对照组(P<0.001),出现尿潴留患者的例数少于对照组(P=0.04)。结论:龙槐止血汤联合咖啡酸片可以加快湿热下注型混合痔术后止血,减轻患者术后水肿、疼痛症状,加快创口愈合,降低术后尿潴留发生率。Objective:To observe the curative efficacy of Longhuai Zhixue Decoction(龙槐止血汤)combined with caffeic acid tablets in the treatment of post-operative complications of mixed hemorrhoids with dampness-heat pouring down syndrome.Methods:A total of 60 patients with mixed hemorrhoids of dampness-heat pouring down syndrome were randomly divided into observation group and control group,with 30 cases in each group.The control group was given caffeic acid tablets and Chinese herbal placebos after surgery,while the observation group was given caffeic acid tablets and Longhuai Zhixue Decoction.Bleeding score,mucosal score,pain score and edema score were measured at 1,3,7 and 14 days after operation.Patients in both groups were followed up on the 30th day after surgery,and the wound healing time was collected.The urinary retention during postoperative recovery was compared between the two groups.Results:After the surgery,the bleeding score and the mucosal score showed a downward trend in both groups,and the bleeding score and the mucosal score in the observation group were lower than those in the control group at 7 and 14 days after surgery(P<0.05).The peak value of the pain score appeared on the 3rd day after surgery in both groups,and then continued to decline.The pain score of the observation group was lower than that of the control group at 3,7 and 14 days after surgery(P<0.05).After the surgery,the edema score of both groups showed a downward trend,and the edema score of the observation group was lower than that of the control group at 7 and 14 days after surgery(P<0.05).The average wound healing time of the observation group was shorter than that of the control group(P<0.001),and the number of patients with urinary retention of the observation group was less than that of the control group(P=0.04).Conclusions:For patients with mixed hemorrhoids of dampness-heat pouring down syndrome,Longhuai Zhixue Decoction combined with caffeic acid tablets can effectively accelerate postoperative hemostasis,reduce postoperati
