摘　　要：目的:观察经脐三通道腹腔镜下离断式肾盂成形术治疗小儿肾盂输尿管连接处梗阻(UPJO)致肾积水的效果。方法:选取2020年7月至2022年9月该院收治的62例UPJO致肾积水患儿进行前瞻性研究,采用随机数字表法将其分为研究组与对照组各31例。对照组采用后腹腔镜离断式肾盂成形术治疗,研究组采用经脐三通道腹腔镜下离断式肾盂成形术治疗,比较两组围术期相关指标水平、肾盏及肾盂分离距离、肾盂前后径、肾小球滤过率及并发症发生率。结果:两组术中出血量、术后胃肠功能恢复时间、住院时间比较,差异无统计学意义(P>0.05);研究组手术时间、引流管拔除时间均短于对照组,差异有统计学意义(P<0.05);术后,两组肾盏及肾盂分离距离均小于术前,但组间比较,差异无统计学意义(P>0.05);术后,两组肾盂前后径均小于术前,肾小球滤过率均高于术前,但组间比较,差异无统计学意义(P>0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:经脐三通道腹腔镜下离断式肾盂成形术治疗UPJO致肾积水患儿可缩短手术时间和引流管拔除时间,其效果优于后腹腔镜离断式肾盂成形术治疗。Objective:To observe effects of transumbilical three-port laparoscopic dismembered pyeloplasty in treatment of hydronephrosis caused by ureteropelvic junction obstruction(UPJO)in children.Methods:A prospective study was conducted on 62 children with hydronephrosis caused by UPJO admitted to this hospital from July 2020 to September 2022.They were divided into study group and control group by using the random number table method,31 cases in each group.The control group was treated with retroperitoneal laparoscopic dismembered pyeloplasty,while the study group was treated with transumbilical three-port laparoscopic dismembered pyeloplasty.The perioperative related index levels,the separation distance between the renal calices and pelvis,the anteroposterior diameter of renal pelvis,the glomerular filtration rate,and the incidence of complications were compared between the two groups.Results:There were no significant differences in the intraoperative blood loss,the postoperative gastrointestinal function recovery time and the hospitalization time between the two groups(P>0.05).The operation time and the drainage tube removal time of the study group were shorter than those of the control group,and the differences were statistically significant(P<0.05).After the surgery,the separation distance levels between the renal calices and pelvis in both groups were smaller than those before the surgery,but there was no statistically significant difference between the groups(P>0.05).After the surgery,the anteroposterior diameters of the renal pelvis in both groups were smaller than those before the surgery,the glomerular filtration rates were higher than those before the surgery;but there was no statistically significant difference between the groups.There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Transumbilical three-port laparoscopic dismembered pyeloplasty for the children with hydronephrosis caused by UPJO can shorten the operation time and the drainage tube re
