血清CYFRA 21-1、AFP/CEA水平联合检测在结直肠癌诊断中的效能

摘　　要：目的:分析血清细胞角蛋白19片段(CYFRA21-1)、甲胎蛋白(AFP)/癌胚抗原(CEA)水平联合检测在结直肠癌诊断中的效能。方法:回顾性分析2019年1月至2023年5月该院收治的75例结直肠病变患者的临床资料,根据病理检查结果将其分为结直肠癌组(n=42)和结直肠良性病变组(n=33),另选取同期75名健康体检者设为对照组。所有受试者均于入院时检测血清CYFRA21-1、AFP、CEA水平,并计算AEP/CEA,比较三组血清CYFRA21-1、AFP、CEA、AFP/CEA水平,绘制受试者工作特征(ROC)曲线分析血清CYFRA21-1、AFP/CEA水平单项及联合检测诊断结直肠癌的效能。结果:结直肠癌组CYFRA21-1、AFP、CEA、AFP/CEA水平均高于结直肠良性病变组和对照组,且结直肠良性病变组高于对照组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,血清CYFRA21-1、AFP/CEA水平单项及联合检测诊断结直肠癌的曲线下面积分别为0.857、0.747、0.895,均具有一定诊断效能,其中联合检测诊断效能最高。结论:CYFRA21-1、AFP/CEA水平联合检测诊断结直肠癌的效能高于二者单项检测。Objective:To analyze efficiency of combined detection of serum cytokeratin 19 fragment(CYFRA21-1)and alpha-fetoprotein(AFP)/carcinoembryonic antigen(CEA)levels in diagnosis of colorectal cancer.Methods:The clinical data of 75 patients with colorectal lesions admitted to this hospital from January 2019 to May 2023 were retrospectively analyzed.According to the results of pathological examination,they were divided into colorectal cancer group(n=42)and colorectal benign lesion group(n=33).The clinical data of 75 healthy subjects in the same period were collected and they were used as the control group.The serum levels of CYFRA21-1,AFP and CEA were detected in all subjects on admission.The levels of serum CYFRA21-1,AFP,CEA and AFP/CEA were compared among the three groups.The receiver operating characteristic(ROC)curve was drawn to analyze the efficiencies of single and combined detection of serum CYFRA21-1 and AFP/CEA levels in the diagnosis of colorectal cancer.Results:The levels of CYFRA21-1,AFP,CEA and AFP/CEA in the colorectal cancer group were higher than those in the colorectal benign lesion group and the control group,those in the colorectal benign lesion group were higher than those in the control group,and the differences were statistically significant(P<0.05).The ROC curve showed that the areas under the curve of single and combined detection of the serum CYFRA21-1 and AFP/CEA levels in the diagnosis of colorectal cancer was 0.857,0.747 and 0.895,separately,all of which had certain diagnostic efficiencies,and the combined detection had the highest diagnostic efficiency.Conclusions:The diagnostic efficiency of combined detection of CYFRA21-1 and AFP/CEA levels in the diagnosis of colorectal cancer is higher than that of single detection.
