延期妊娠初产妇引产后剖宫产的影响因素分析

摘　　要：目的分析延期妊娠初产妇引产后剖宫产的影响因素。方法选取2020年1月至2021年1月本院接收的100例延期妊娠初产妇作为研究对象,统计初产妇引产后的分娩方式,采用单因素和多因素Logistic回归分析延期妊娠初产妇引产后剖宫产的影响因素。结果100例初产妇中,引产后剖宫产分娩28例(28.00%),引产后阴道分娩72例(72.00%)。引产后剖宫产分娩产妇与引产后阴道分娩产妇年龄、产时体重指数、临产时宫口扩张、人流次数、胎儿性别、孕龄、胎儿体质量、人工破膜、脐带绕颈比较差异无统计学意义;引产后剖宫产分娩产妇与引产后阴道分娩产妇身高、羊水粪染、引产前Bishop评分、产时催产素应用情况及潜伏期比较差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,身高<160 cm、引产前Bishop评分<6分、潜伏期时间>15 h为影响延期妊娠初产妇引产后剖宫产的独立危险因素(OR>1,P<0.05)。结论身高<160 cm、引产前Bishop评分<6分、潜伏期时间>15 h是影响延期妊娠初产妇引产后剖宫产的独立危险因素,临床需提高重视,进一步做好延期妊娠初产妇的引产前评估工作,以帮助其选择合适的分娩方式。Objective To analyze the influencing factors of cesarean section after induction of labor in primipara with delayed pregnancy.Methods 100 cases of primiparas with delayed pregnancy received in our hospital from January 2020 to January 2021 were selected as the research subjects,and the delivery mode of primiparas after induction of labor was counted,univariate and multivariate Logistic regression analysis was used to analyze the influencing factors of cesarean section after induction of labor in primiparas with delayed pregnancy.Results Among the 100 primiparas,28 cases(28.00%)had cesarean section after induction of labor,and 72 cases(72.00%)had vaginal delivery after induction of labor.There were no significant differences in age,body mass index during delivery,uterine orifice dilation during labor,number of abortions,fetal gender,gestational age,fetal body mass,artificial rupture of membranes,umbilical cord around the neck between the primiparas with cesarean section and vaginal delivery after induction of labor;there were significant differences in height,fecal staining of amniotic fluid,Bishop score before labor induction,application of oxytocin during labor and incubation period between the primiparas with cesarean section and vaginal delivery after induction of labor(P<0.05).The results of multivariate Logistic regression analysis showed that height<160 cm,Bishop score<6 scores before induction of labor,and incubation period>15 h were independent risk factors for cesarean section after induction of labor(OR>1,P<0.05).Conclusion Height<160 cm,Bishop score<6 scores before induction of labor,and incubation period>15 h are independent risk factors affecting cesarean section after induction of labor in primiparas with delayed pregnancy,which needs to be paid more attention in clinical attention,to further evaluation the pre-induction of labor in primiparas with prolonged pregnancy to help them choose the right delivery method.
