孕早中期MMP-9、NLR、Hb联合子宫螺旋动脉超声在早发型痫前期预测中的应用价值

摘　　要：目的探讨孕早中期外周血基质金属蛋白酶-9(MMP-9)、中性粒细胞计数与淋巴细胞计数比值(NLR)、血红蛋白(Hb)联合子宫螺旋动脉超声在早发型子痫前期(PE)预测中的应用价值。方法选取2021年1—12月本院收治的45例早发型PE孕妇作为观察组,另选取本院同期45名单胎健康孕妇作为对照组,均行子宫螺旋动脉超声检查及血清MMP-9、NLR、Hb水平检测。比较两组血清MMP-9、NLR、Hb水平,绘制ROC曲线并计算AUC分析血清MMP-9、NLR、Hb和子宫螺旋动脉超声及联合诊断对早发型PE的预测价值。结果观察组血清MMP-9、Hb水平均低于对照组,NLR水平高于对照组,差异有统计学意义(P<0.05)。血清MMP-9、NLR、Hb、子宫螺旋动脉超声、联合诊断的AUC分别为0.769、0.756、0.742、0.744、0.895,均具有一定预测价值,但联合诊断的预测价值更高。结论孕早中期MMP-9、NLR、Hb联合子宫螺旋动脉超声在早发型PE预测中应用价值显著,值得临床推广应用。Objective To explore the value of peripheral blood matrix metalloproteinase-9(MMP-9),neutrophil-to-lymphocyte ratio(NLR),hemoglobin(Hb)combined with uterine spiral artery ultrasound in the prediction of early-onset preeclampsia(PE)in the early and middle trimester of pregnancy.Methods 45 pregnant women with early-onset PE treated in our hospital from January 2021 to December 2021 were selected as the observation group,and another 45 healthy pregnant women with singleton in our hospital during the same period were selected as the control group,all of them underwent uterine spiral artery ultrasound and levels of serum MMP-9,NLR and Hb were detected.The levels of serum MMP-9,NLR and Hb were compared between the two groups,ROC curve was drawn and AUC was calculated to analyze the predictive value of serum MMP-9,NLR,Hb and uterine spiral artery ultrasound and combined diagnosis for early-onset PE.Results The levels of serum MMP-9 and Hb in the observation group were significantly lower than those in the control group,and the level of NLR was significantly higher than that in the control group,the differences were statistically significant(P<0.05).The AUC values of serum MMP-9,NLR,Hb,uterine spiral artery ultrasound and combined diagnosis were 0.769,0.756,0.742,0.744 and 0.895,respectively,all of which had certain predictive value,but the predictive value of combined diagnosis was higher.Conclusion The application value of MMP-9,NLR and Hb combined with uterine spiral artery ultrasound in early and middle trimester of pregnancy has significant application value in the prediction of early-onset PE,which is worthy of clinical promotion and application.
