枣仁安神胶囊对老年心绞痛合并糖尿病患者PCI围手术期焦虑、抑郁及MACE的影响研究

摘　　要：目的:探讨枣仁安神胶囊对老年心绞痛伴糖尿病患者经皮冠状动脉介入治疗(PCI)围手术期焦虑、抑郁和术后主要不良心血管事件(MACE)的影响。方法:选取2019年5月至2022年5月该院择期行PCI的老年心绞痛伴糖尿病患者142例,采用随机数字表法分为对照组和观察组,每组71例。对照组患者在围手术期予以常规治疗,观察组患者在对照组的基础上予以枣仁安神胶囊,疗程为4周。比较两组患者围手术期汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分,焦虑症和抑郁症发生率,血清髓过氧化物酶(MPO)、5-羟色胺(5-HT)水平,3个月内MACE发生率、社会功能缺陷筛选量表(SDSS)评分和健康调查简表(SF-36)评分及不良反应发生情况。结果:观察组患者的HAMA评分、HAMD评分在术前第1日、术后第3日、术后第7日、术后第14日和术后第28日均显著低于对照组,差异均有统计学意义(P<0.05)。对照组患者围手术期焦虑、抑郁发生率分别为26.76%(19/71)、19.72%(14/71),观察组患者围手术期焦虑、抑郁发生率分别为7.04%(5/71)、2.82%(2/71),组间差异有统计学意义(P<0.05)。治疗后,观察组患者5-HT水平明显高于对照组,MPO水平明显低于对照组,差异均有统计学意义(P<0.05)。术后3个月内,观察组、对照组患者的MACE发生率分别为9.86%(7/71)、28.17%(20/71),差异有统计学意义(P=0.005)。治疗后,观察组患者SDSS评分显著低于对照组,SF-36评分显著高于对照组,差异均有统计学意义(P<0.01)。两组患者均未发生明显不良反应。结论:枣仁安神胶囊能显著降低老年心绞痛伴糖尿病患者PCI围手术期的焦虑、抑郁发生率和MACE发生率,提高社会功能状态和生活质量。OBJECTIVE:To probe into the effects of Zaoren Anshen capsules on perioperative anxiety and depression and major adverse cardiovascular events(MACE)of percutaneous coronary intervention(PCI)in senile patients with angina complicated with diabetes.METHODS:Totally 142 senile patients with angina complicated with diabetes underwent PCI from May 2019 to May 2022 were selected to be randomly divided into control group and observation group via random number table method,with 71 cases in each group.The control group was given conventional treatment during perioperative period,while the observation group received Zaoren Anshen capsule basis on the control group,the course of treatment was 4 weeks.Scores of Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD),incidences of anxiety disorder and depression disorder,levels of serum myeloperoxidase(MPO)and serotonin(5-HT)during perioperative period,the incidences of MACE,scores of Social Disability Screening Schedule(SDSS)and Short Form 36 Health Survey(SF-36),incidences of adverse drug reactions within 3 months were compared between two groups.RESULTS:The HAMA score and HAMD score of the observation group were significantly lower than those of the control group on the day before surgery,the 3rd day,the 7th day,the 14th day and the 28th day after surgery,with statistically significant differences(P<0.05).The incidences of perioperative anxiety and depression of the control group were respectively 26.76%(19/71)and 19.72%(14/71),the incidence of perioperative anxiety and depression were respectively 7.04%(5/71)and 2.82%(2/71),with statistically significant differences(P<0.05).After treatment,the 5-HT level was significantly higher and the MPO level was significantly lower in the observation group than those in the control group,with statistically significant differences(P<0.05).Within 3 months after PCI,the incidences of MACE in the observation group and control group were respectively 9.86%(7/71)and 28.17%(20/71),the difference was statistically significant(P=0.005)
